Guidelines of Application Form for the JICA Training and Dialogue Program JICA技术型进修和对话型进修项目申请表填写说明 The attached form is to be used to apply for the training and dialogue programs of the Japan International Cooperation Agency (JICA), which are implemented as part of the Official Development Assistance Program of the Government of Japan. Please complete the application form while referring to the following and consult with the respective country’s JICA Office - or the Embassy of Japan if the former is not available - in your country for further information. 此附属表格作为日本政府实施贯彻政府援助计划的一部分，适用于申请JICA日本国际协力机构技术型进修和对话型进修项目。请参照下列说明，并咨询JICA驻外事务所或日本大使馆(如果所在国没有JICA事务所)完成此申请表。 1. Parts of Application Form to be completed（需要完成的申请表部分） 1) Which part of the form should be submitted? （哪一部分表格需要提交？） It depends on the type of training and dialogue program you are applying for. （取决于你所申请的进修项目的类型） Application for Group and Region Focused Training Program 申请集团进修项目 Official application and Parts A and B including Medical History and Examination must be submitted. （必须提交官方申请、A部分、B部分以及健康状况申报表） Application for Country Focused Training Program including Counterpart Training Program 申请国别进修项目包括对口人员进修项目 Part B including Medical History and Examination will be submitted. Official application and Part A need not to be submitted （提交B部分以及健康状况申报表。官方申请和A部分不需提交。） 2) How many parts does the Application Form consist of?（申请表由几部分组成） The Application Form consists of three parts as follows; （此申请表由以下三部分组成） Official Application （官方申请） This part is to be confirmed and signed by the head of the relevant department/division of the organization which is applying. （此部分由申请单位/部门领导确认和签字） Part A.
