急性胆源性胰腺炎和胰胆管合流异常的关系 　　作者：陈风，汪健，蒋晓飞，徐卫燕，蔡雷，黄顺根 作者单位：1.上海南汇医院 普外科，上海 201300;2.苏州大学附属儿童医院 普外科，江苏 苏州 215003 　　【摘要】 目的 探讨急性胆源性胰腺炎(ABP)和胰胆管合流异常(APBDU)的关系。方法 选择ABP手术患者165例，发病时抽取静脉血检测总胆红素(TB)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)，谷氨酰转移酶(GGT)，保守治疗后复查;术中胆道造影观察胰胆管合流情况。结果 165例胆道造影患者中，发现APBDU 41例。保守治疗后APBDU和NAPBDU组的TB、ALT、AST、ALP、GGT都明显降低，有统计学意义(P0.05)。治疗后TB、ALP都恢复正常，APBDU组仍高于NAPBDU组，但没有统计学意义(P0.05);APBDU组的ALT、AST和GGT都高于NAPBDU组，有统计学意义(P0.05)。结论 APBDU是导致ABP的重要原因之一。 　　【关键词】 胆源性胰腺炎，胰胆管合流异常，术中胆道造影 　　Study on the relationship between acute biliary pancreatitis and anomalous pancreaticobiliary ductal union CHEN Feng*， WANG Jian， JIANG Xiaofei*， et al. * The Hospital of Nanhui， Shanghai 201300 　　Abstract Objective To study the relationship between acute biliary pancreatitis and anomalous pancreaticobiliary ductal union. Methods One hundred and sixty five patients with acute biliary pancreatitis undergone surgery were performed to test total bilirubin (TB)， alanine amintransferase (ALT)， aspartate amintransferase (AST)， alkaline phosphates (ALP)， glutamyl transferase (GGT) when they had symptoms. After being treated， they were tested again. Then the pancreaticobiliary ductal union was observed through intraoperative cholangiography. Results Fouty one patients with anomalous pancreaticobiliary ductal union were found in 165 patients with intraoperative cholangiography. A significant decrease was observed which included that alanine amintransferase (ALT)， aspartate amintransferase (AST)， alkaline phosphates (ALP)， glutamyl transferase (GGT) after treated in non surgery in both group of APBDU and NAPBDU (P0.05). Although total bilirubin and alkaline phosphates reduced to the normal values， the values of APBDU were higher than those of NAPBDU， there was no statistical significance (P0.05). However， alanine amintransferase (ALT)， aspartate amintransferase (AST) and glutamyl transferase (GGT) were higher in group of APBDU than that in NAPBDU with statistical difference (P0.0
