In case of Public Limited Company or Partnership firm the legal deed of its constitution be attached. 6. Details of manufacturing licence etc. with attested copy. 7. Photo copy of Registration Certificate of the Company. 8. Sales Tax/ TIN registration no. (Enclosed attested copy of the Registration Certificate) 9. Excise registration no. - If applicable, please specify if the product is exempted. (Enclose attested copy of the Registration Certificate) 10. Test report from a Govt. approved/ NABL accredited laboratory, not older than 6 months old, certifying that the product is fit for human consumption for food items and that the product confirms to specifications and standards as per BIS norms mentioned in Annexure-1. (Attested copy of test reports for all brands and variety to be submitted) 11. Valid license issued by Competent Authority-Health license/ PFA/FPOI/ ISI etc. (enclosed attested copies of the necessary licenses) 12. Shelf life of the product (s). 13.
