Individual Stress Risk Assessment This assessment aims to identify any stress problems and areas (stressors), develop and implement solutions to the problems and monitor their effectiveness. Time should be set aside between the assessor (ideally the line manager) and employee to complete this assessment. The team/department risk assessment (if completed) may also be used for reference. The employee’s “My Performance” may also indicate if there are any further issues arising that need to be discussed. How to complete: Each area in section 1 should be discussed and if identified as a stressor carried forward onto the action plan. The action plan (section 2) identifies the stressors highlighted by the individual, areas that are causing stress, current support and any further support that may need to be put in place. The employee needs to rate how stressful they find the stressor on a scale of 0-10 on the main action plan. This gives the risk assessor an idea of the degree of stress being experienced by the employee. The risk assessment needs to be agreed and signed by the employee and assessor. The review action plan needs to be completed after 3 to 6 months initially. Further reviews may be necessary after this time however it is recommended that the review action plan is discussed at least annually. The employee should complete the review stress rating in the review action plan to allow a comparison to the initial stress rating score in the main action plan. A copy of the risk assessment and action plan should be held by the assessor and employee.
