单一免疫法与联合免疫法检测便隐血的临床应用评价 蔡力力1 白 洁1 林 凯2 李小霞1 丁海琴2 1解放军总医院南楼检验科 北京 100853 2 空军总医院临床检验中心 北京 100036 【摘要】 目的:观察两种便隐血实验方法—单一免疫法（胶体金单克隆抗体血红蛋白检测法）和联合免疫法（血红蛋白、转铁蛋白联合免疫法）检测的灵敏性、特异性和抗干扰性，以便在工作中避免假阳性和假阴性反应。[方法] 1、单一免疫法和联合免疫法检测人7种不同浓度血红蛋白液对比灵敏度。2、取人和常食用的6种动物血红蛋白检测单一免疫法和联合免疫法的特异性和准确度。3、比较单一免疫法和联合免疫法对107例经内窥镜检查确诊消化道出血患者的阳性检出率。 [结果] 单一免疫法和联合免疫法的灵敏性和特异性相同，联合免疫法的稳定性和抗干扰性优于单一免疫法，单一免疫法可出现假阴性和假阳性反应。[结论] 联合免疫法稳定性强，是目前检测便隐血的较好的方法，将转铁蛋白作为补充检测手段是临床判断是否存在出血的最有价值的方法。 【关键词】便隐血实验 单克隆抗体 血红蛋白 转铁蛋白 The comparison of the mono-clone antibody assay and the united immunoassay for fecal occult blood tests and their clinical evaluation Cai Lili1 ，Bai Jie1，Lin Kai2 ，Li Xiaoxia1，Ding Haiqing2 1Department of Clinical Laboratory ，General Hospital of PLA ，Beijing 100853 2 Clinic Laboratory Center ，Air Force Hospital，Beijing 100036 [Abstract] Objective: To observe the sensitivity ,specificity and ability of anti-interference of two assays for fecal occult blood in order to avoid false-positive reaction and false-negative reaction.One test is the mono-clone antibody assay ,the other is hemoglobin and transferrin united immunoassay . [Methods] 1、 To compare the sensitivity among mono-clone antibody assay and united immunoassay by testing hemoglobin from human beings with seven different concentrations. 2、The specificity of the mono-clone antibody assay and the united immunoassay were detected with human and six different kinds of animals. 3、The mono-clone antibody assay and the united immunoassay were used to detect dung occult blood from 107 patients that were diagnosed as diseases of alimentary canal and then compared their positive rate of two methods .[Results] The mono-clone antibody assay and the united immunoassay have the same sensitivity and specificity ，the united immunoassay has higher ability anti-interference than the mono-clone antibody assay. the mono-clone antibody assay can appear false-negative reaction and false-positive reaction.
