* 临床医生最希望的是什么？是选择一种恰当的抗感染药物，治疗病原体感染，取得好的疗效。 但在临床实际过程中，选择的抗感染药物常常是不恰当的，尽管这不是医生所希望的。 如果让大家报出引起院内获得性肺炎最常见的五种主要的病原体，相信大家都非常熟悉。 那么，这五种主要病原体的耐药情况如何呢？ Kollef的 研究，显示了在常见的病原体感染当中，不恰当的抗感染药物治疗导致的耐药让人非常惊讶。 因此今天的重要信息之一是：在感染性疾病的治疗中，不仅要了解主要病原体的流行情况，更重要的是它的耐药情况。 即便在同样的医疗机构，不同的医疗科室如内科和外科ICU，同样的病原体其耐药情况也可能不完全相同。 * * * 这篇文章继续关注大肠杆菌耐药的问题。上面的这张图，第1至4个柱子，是细菌呈现的耐药性的不同，第1个柱子的细菌是全敏感的，第2个柱子的细菌对某一类抗生素耐药，第3根柱子的细菌对某两类抗生素耐药，第4根柱子的细菌对三类以上的抗生素耐药，随着耐药的增加，病人接受不正确抗菌药物治疗的机率就明显增加。 下面的这张图是对比病死率的情况，随着细菌耐药性的不断增加，病死率也是明显的增加。 * 除了抗感染的药物选择河使用时间的恰当问题以外，这几年我重点在考虑，如何优化抗感染药物治疗方案，包括给药剂量、给药频率和给药持续时间。 Inadequate treatment of hospital-acquired pneumonia, particularly in the ICU setting, is increasingly being recognized as a potential cause of patient mortality and morbidity.
