自由体位分娩在第二产程中的应用效果 　　[摘要] 目的 研究自由体位分娩在第二产程中的应用效果。 方法 选择2014年1～6月来本院分娩的55例产妇为观察组，在第二产程采取自由体位；同时选择同时期产妇55例为对照组，采取常规仰卧截石位，对比观察两组的效果。 结果 观察组的第二产程时间明显短于对照组（P0.05）；观察组的产后大出血、产后不适及新生儿窒息率均明显低于对照组（P0.05）；两组的会阴损伤情况差异有统计学意义（P0.05）；观察组的产妇满意度明显高于对照组（P0.05）。 结论 产妇在第二产程采用自由体位分娩不仅可缩短第二产程时间，降低产后大出血、产后不适及新生儿窒息率，而且还能提高产妇满意度，值得临床推广应用。 　　[关键词] 第二产程；自由体位；常规仰卧截石位；新生儿窒息；产后大出血 　　[中图分类号] R714.3 [文献标识码] A [文章编号] 1674-4721（2015）08（b）-0077-03 　　Application effect of delivery with liberal body position at the second stage of labor 　　SUN Qin TANG Jing 　　Maternity Ward，Wuzhong，Wuzhong People′s Hospital of Suzhou City，Suzhou 215128，China 　　[Abstract] Objective To study the application effect of delivery with literal position at the second stage of labor. Methods From January to June 2014，55 pregnant women delivered in our hospital were selected as observation group，and they were given liberal body position at the second stage of labor.During the same period，55 pregnant women were selected as control group，and they were given conventional supine lithotomy position.The effect between two groups was observed and compared. Results Time of the second stage of labor in observation group was obviously shorter than that of control group （P0.05）.The rate of postpartum massive hemorrhage，postpartum discomfort and neonatal asphyxia in observation group was obviously lower than that of control group respectively （P0.05）；there was a statistical difference of perineal injury between two groups （P0.05）；satisfaction degree of pregnant women in observation group was higher shorter than that of control group （P0.05）. Conclusion For pregnant women，adoption of liberal position for delivery at the second stage of labor can not only shorten the time of the second stage of labor，decrease the rate of postpartum massive hemorrhage，postpartum discomfort and neonatal asphyxia，but also improve satisfaction degree of pregnant women.It is worthy of clinical promotion and application. 　　[Key words] Second stage of lab
