三种不同内固定治疗胫骨平台后外侧骨折的生物力学研究 李勇1，2 龚泰芳1 蔡林2邓州铭2 陈文1 柯文1 1湖北医药学院附属太和医院骨关节科 2 武汉大学中南医院骨科 【摘要】 目的 比较三种不同内同定方式治疗胫骨平台后外侧骨折的生物力学性能。方法 将18具经福尔马林溶液固定后的成人尸体膝关节标本，根据胫骨平台后外侧剪应力骨折的形态学特征建立后外侧骨折模型，随机分成3组 n 6 ：A组采用前外侧5孔T型有限接触动力加压钢板 LC-DCP 固定，B组采用后外侧直型7孔重建接骨板固定，C组采用后外侧胫骨平台锁定钢板固定。测量三组在轴向载荷500N、800N、1000N、1200N、1500N、2000N、2300N下的垂直位移、失效载荷和应变数据。 结果1．A、B、C三组在500N载荷下骨折块的相对位移分别为 ±0.073 、 0.8±0.092 、 0.±0.0270 mm，在1000 N载荷下骨折块的相对位移分别为 1.±0.059 、 0.±0.037 、 0.±0.166 mm，在1500 N载荷下骨折块相对位移分别为 2.2±0.073 、 1.518±0.1 、 0.70±0.058 mm。 C组与其它2组比较差异均有统计学意义 P 0.05 。B组与A组比较差异有统计学意义 P 0.05 。2．A、B、C三组失效载荷分别为 1±52.697 、 ±162.555 、 ±71.267 N，C组与其它2组比较差异均有统计学意义 P 0.05 。B组与A组比较差异有统计学意义 P 0.05 。3．A组载荷-应变曲线变化规律不一致；B组载荷-应变曲线变化规律不太一致，相差较大；C组载荷-应变曲线变化规律一致，相近。结论：胫骨平台后侧骨折使用新的内固定器械-胫骨平台后侧锁定钢板良好的生物力学。Biomechanical analysis of three fixations for the posterolateral shearing tibial plateau fracture.LI Yong, CAI Lin, GONG Tai-fang. 【Abstract】Objective The study of biomechanical strengths of three different fixations of the posterolateral sheafing tibial plateau fracture. Methods The posteolateral shearing fracture models were created in eighteen single of embalmed cadavers of adults around a tibial specimens and assigned into three groups randomly with six single in each group. Each fracture model was randomly instrumented with one of the three following constructs, an anterolateral limited contact dynamic compression plate LC-DCP , a posterolateral straight type buttress plate, or a posterolateral locking plate. Vertical displacements of the posterolateral fragments and failure loads and strain data were measured under axial loads 500N to 2300N. Results 1.The vertical displacements in groups A, B and C were respectively 0.898±0.073 mm, 0.418±0.092 mm, 0.203±0.0270 mm under axial loads of 500N; 1.658±0.059 mm, 0.801±0.037 mm, 0.536±0.166 mm under 1000N; 2.271±0.073 mm, 1.518±0.109 mm, 0.780±0.058 mm under 1500N; The differences between the 3 groups were statistically significant P 0.05 . groups wer
