Johnston County 4-H ’09 Summer Fun Schedule  Activities are open to all youth specific to the age appropriateness of the activities. Youth do NOT have to be in 4-H to participate. However, these events or activities operate under the 4-H Code of Conduct and Disciplinary Procedure, located on the back of this document. The 4-H Code of Conduct and Disciplinary Procedure is a condition of participation in any North Carolina 4-H activity or program. TO REGISTER: Fill out the registration form and return it with your completed, **notarized Medical Release form and payment, checks should be made payable to Johnston County 4-H and dropped off at the 4-H Office or mailed to Johnston County CES, 4-H Summer Fun Program, c/o Angie Hampton at 2736 NC HWY 210, Smithfield, NC 27577. One registration form for each participant is sufficient for multiple activities. Check each activity you register for on the form. You may call to see if space is available, but registration will NOT be taken by phone or email. Youth will be placed on waiting lists after camps fill up and notified if space becomes available. NO REFUNDS WILL BE GIVEN UNLESS THE ACTIVITY IS CANCELED BY THE 4-H OFFICE. 4-H members may register on a first-come, first-serve basis, pending payment of registration fee. A need-based scholarship form is available upon request. All returned checks will have a service charge of $25.00 each. There will be a sign-in and sign-out procedure at camps that everyone must follow…for safety. Volunteers are needed at some of the camps.
