儿童可取出式覆膜食管支架置入术后常见并发症的预防及处理 罗刚, 刘新献, 黄穗, 刘帆★, 马琦 【摘要】 目的 探讨可取出式覆膜支架治疗儿童良性食管狭窄常见并发症的发生因素、预防与处理。方法15例经食管造影确诊的良性食管狭窄患儿在透视下行支架置入术，全过程顺利，并于2-3周将支架取出，术后追踪观察其常见并发症的发生，并采取积极有效的措施进行相关预防及处理。结果 全部病例支架置入一次成功，放置支架后患者的饮食均有明显的改善，术后随访1、2、3周，3月，总结其并发症主要为支架脱落、移位5例，采用牵拉固定法能防止移位和脱落；食管再狭窄2例；食管-气管瘘1例；术后胸骨后钝痛及异物感6例；以及胃食管反流，上消化道出血等。结论 儿童覆膜食管支架置入术操作简便、安全、有效，术后并发症不容忽视，积极处理和预防可减少并发症的发生，对改善患儿的预后有着明显的优势。 【关键词】 覆膜食管支架；儿童良性食管狭窄；并发症；预防及处理 中图分类号：R814.43 文献标志码：A 文章编号： Prevention and treatment of common complications after esophageal stent placement for children LUO Gang,LIU Xin-xian,HUANG Sui,LIU Fan ★,MA Qi. Methods 15 patients with benign esophageal stenosis were diagnosed by barium of carotid stenting perspective under X-ray,the whole process of smoothly,and for 2-3 weeks will support,follow-up observation of the common complications, and take positive and effective measures for prevention and treatment.observe the complications after operation.Results all cases were successful stent implantation,the patients diet were significantly improved after operation，postoperative follow-up of 1, 2, 3 weeks,3month,summed up the main complications as stents,shift in 5 cases, using the distraction fixation can prevent the shift and off; Esophageal restenosis in 2 cases;esophageal-tracheal fistula in 1 case;Postoperative sternal dull pain and foreign body sensation after 6 cases;And gastroesophageal reflux, upper gastrointestinal bleeding,etc.Conclusion children covered esophageal stent is simple,safe,effective,but its complications can not be ignored,treatment and prevention can reduce complications actively,has obvious advantage for improving the prognosis of children. 【Key words】covered esophageal sten
