肺炎克雷伯菌的科室分布分布和耐药性分析 常美云 摘要 目的了解西山煤电古交矿区总医院临床分离149株肺炎克雷伯菌的细菌病原菌分布和耐药情况。方法 采用法国梅里埃API板条鉴定，K-B纸片扩散法对临床分离的149株肺炎克雷伯菌进行抗生素敏感性测定。结果 肺炎克雷伯菌科室分布广泛，149株肺炎克雷伯菌产ESBLs肺炎克雷伯菌53株,检出率为35.57 %。149株菌株对亚胺培南全部敏感,其余药物呈现不同程度的耐药。结论 重视产ESBLs肺炎克雷伯菌的耐药性监测,根据药敏试验结果合理使用抗生素。 [关键词]超广谱β内酰胺酶；细菌耐药性；肺炎克雷伯菌；药敏试验 Distribution and Antibiotic Resistance of Klebsiella pneumoniae ChangMei-yun （General Hospital of Xishan Coal and Electricity ,GuJiao 030200) Abstract objective to investigate the department of distribution and the resistance of149 clinical isolates of klebsiella pneumonia in .GeneralHospital of Xi shan Coal and Electricity. Methods Collect 149cases from GuJiao general of hospital from January toDecember, 2010, use API strips to identify positive strains, and take the drug sensitive test with K-B paper strips diffusion method. Results Klebsiella pneumoniae of infection are widely distributed,there are 53strain producing ESBLS inthe149Klebsiellapneumoniaethe rate of 35.57% bacteria.All? strains were sensitive to imipenem, the rest of strain showeddifferent degree of resistance drug .Conclusions It is important to surveillance producting ESBLS ofresistance drug of bacteria, according to drug susceptibility test results , antibiotics should be used rationally. [Key words] extended spectrum β-lactamase; bacterial resistance; Klebsiella pneumoniae ；Antimicrobial susceptibility test 　 肺炎克雷伯菌是肠杆菌科常见的条件致病菌,随着第三代头孢菌素等广谱抗菌药的广泛使用,产超广谱β-内酰胺酶(extended spectrum-lactamase ,ESBLs) 菌株不断增多,成为临床治疗的难题之一。对2010年住院患者分离出的肺炎克雷伯菌149株进行了检测,回顾性分析肺炎克雷伯菌病原菌分布科室和药敏结果,并对产ESBLs与不产ESBLs 菌株耐药情况进行分析比较。 1.1 材料与方法 1.1.1菌种来源　收集2010年1月1日至2010年12月31日的住院患者各种标 本分离的肺炎克雷伯菌,剔除同一部位重复的菌株。 1.1.2 细菌鉴定和药敏试验　采用法国梅里埃API板条进行鉴定，药敏试验用K-B纸片法进行。质控菌株:大肠埃希菌ATCC25922、肺炎克雷伯菌ATCC700603 (卫生部临床检验中心)，各种药敏纸片均为OXOID公司产品, 在有效期内使用并进行严格质控。药敏判定标准采用美国临床实验室标准化委员会NCCLS 2009年[1]推荐的方法。 1.1.3 药敏结果用用WHONET 5.4软件进行统计分析结果。 1.1.4 ESBLs确证实验： 按标准纸片扩散法的规定进行，同时使用头孢他啶(30μg)和头孢他啶/克拉维酸(30/10μg)及头孢噻肟(30μg)和头孢噻肟/克拉维酸(30/10μg)两对药敏纸片，当两个药物中有任何一个在加克拉维酸后抑菌环直径与不加克拉维酸的抑菌环
