过敏性紫癜与呼吸道病毒的关系：病因学的探索 李 莎1 董丽群2 王 峥2 冯仕品1 （1. 成都市儿童医院儿肾科,四川成都 610017；2. 四川大学华西第二医院儿肾科，四川成都 610041） [摘要] 目的 探讨呼吸道病毒感染在过敏性紫癜发病机制中的作用。 方法 本文统计了2006年1月至2008年12月在成都市儿童医院及四川大学华西附二院住院的全部1533例过敏性紫癜,了解其季节分布特点，采用酶联免疫吸附法观察40例过敏性紫癜 血浆中呼吸道合胞病毒、腺病毒、流感病毒、副流感病毒IgM抗体好发在寒冷季节。2．血浆 过敏性紫癜组呼吸道病毒阳性22例(22/40,55%),其中副流感病毒的检出率（12/40,30%）高于呼吸道感染组（1/42,2.4%），有统计学差异（X2=11.715，P＜0.05），呼吸道合胞病毒、腺病毒、流感病毒的检出率二组之间无统计学差异（P＞0.05） 3. 外周血单个核细胞 过敏性紫癜组呼吸道病毒阳性24例(24/40,60%),其中副流感病毒的检出率（12/40,30%）高于呼吸道感染组（1/42,2.4%）有统计学差异（X2=11.715，P＜0.05），呼吸道合胞病毒、腺病毒、流感病毒A、流感病毒B检出率二组之间无统计学差异（P＞0.05）4. 肾组织 呼吸道病毒抗原13例阳性（13/40，32.5%），其中副流感病毒12例(12/13) (副流感病毒II10例，副流感病毒III 2例)，1例为呼吸道合胞病毒。抗原在肾小管的荧光强度及密度明显高于肾小球。5. A组及A1组24小时尿蛋白、微量白蛋白分别高于B组及A2组(P0.05),.A组与B组,A1组与A2组在病变程度（ISKDC分级）、免疫复合物沉积部位、免疫病理类型、肾小管萎缩变性、肾间质病变、毛细血管内皮增生、Bowman,s囊改变之间无统计学差异。结论 呼吸道病毒,特别是副流感病毒感染可能与过敏性紫癜发病有关。 关键词 呼吸道病毒；过敏性紫癜； Correlation of repiratory virus and Henoch-Schonlein purpura :an exploring on etiology Li Sha,Wang Zheng, Dong Liqun , Feng Shipin Department of Pediatrics; West China Second Hospital; Sichuan University,chengdu 610041 Abstract Objective To explore the role of respiratory tract virus infection in the pathogenesis of HSP. Methods We analyzed seasonal trends of Henoch-Schonlein Purpura at the Childrens Hospital of Chengdu and West China Second University Hospital over two years. In 40 of the 1533 cases of Henoch-Schonlein Purpura and in 42 matched-controls, we tested for the presence of parainfluenza virus , influenza virus , respiratory syncytial virus and adenovirus in plasma, peripheral blood mononuclear cells, and renal tissue.Results Henoch-Schonlein Purpura mainly occurred in cold weather. In plasma and peripheral blood mononuclear cells , there were no significant differences in the presence of viruses between the Henoch-Schonlein Purpura and control groups. However, the rate of parainfluenza virus infection was significantly higher in the plasma and peripheral blood mononuclear cells of Henoch-Schonlein Purpura
