±望里兰丝堡垫垄!!!!釜笙!!鲞笙!塑￡垫i呈』坚型!璺!受!g!!!!翌!!：!!!!：∑!!!!：型!!·435·◆◆．中枢神经影像学CT乙 inl perfusion’tusionimaging combied witllith CT1 ang ography lassessment of cerebral perfusion and impact ofWillis circle structure in patients with internal carotid artery or middle cerebral artery stenosisYAN Li—hui，DAI Zhen—yu’，YA0 Li—zheng，LJ Xin，ZHOU Gui—long(Department of Radiology，YanchengHospital，Southeast University，Yancheng 224001，China)[Abstract]Objective To assess the cerebral perfusion and the impact of Willis circle structure in patients with internal carotid artery(ICA)or middle cerebral artery(MCA)stenosis using CT perfusion(CTP)imaging combined with CT八Methods Totally 42 patients with unilateral carotid artery stenosis screened by transcranial doppler(TCD)underwent combined CTP and CTA examination．The site and extent of stenosis，structure of Willis circle and blood perfusion werestudied．R馏mts Abnormal signs were observed in all patients，including ICA stenosis in 29 cases and MCA stenosis in 1 3 cases．Mild，moderate and severe stenosis was found in 11，13 and 14 cases，respectively，and occlusion was found in 4 ca— ses．There was no significant difference of perfusion parameters between bilateral vessels in mild stenosis cases(an P 0．05，．MTT and TTP of stenosis side vessel were higher than those of contralateral side in moderate stenosis cases(all PO．05)．In severestenosis Orobstruction cases，CBV，M1vr andTTP of stenosis sidevessel were higher thanthose ofcontralateralside，while CBF of stenosisside vessel waslower than contralateral side(an PO．05)．Abnormalcerebralperfusion was observed in 2 1 patients．Significant statistical difference of abnormal cerebral perfusion classification was found between ICA and MCA stenosis patients．Conclusion CTP combined with CTA iS an effective method tO assess tcerebralperfusion and the structure of Willis circle in patients with ICA or MCA stenosis．[1【ey words]Tomography，X-ray computed；Perfusion imaging；willis circle；Hemodynamies，CT灌成像
