·142·Chinese Journal of Otology Vol. 7, No.2, 2009·临床研究·360°滚转复位法治疗良性阵发性位置性眩晕单希征孙勍马丽涛李娜李红云赵龙珠武警总医院耳鼻咽喉头颈外科（北京100039）【摘要】 目的明确在后半规管平面 360° 旋转患者对治疗典型的后半规管良性阵发性位置性眩晕是否有效。方法回顾分析了 46 例后半规管良性阵发性位置性眩晕患者， 其中 25 例采用三维滚轮 360° 滚转复位法进行复位治疗， 21 例采用 Epley 手法复位治疗， 对其疗效进行比较。 结果 采用三维滚轮 360° 滚转复位法治疗的患者中第 一次治疗后有 23 例患者 （92%） 治愈， 2 例患者经过第二次治疗治愈； 采用 Epley 手法复位的患者中第一次治疗后有 19 例患者 （91.5%） 治愈， 2 例患者经过第二次治疗治愈。 应用 χ2 检验 （α = 0.05， χ2 = 0.1173， P = 0.7319） 二者的差异没有统计学意义。结论三维滚轮 360° 滚转复位法能有效的治疗后半规管良性阵发性位置性眩晕， 其成功率及效果与 Epley 法一致， 且操作简便、 舒适度优于传统方法。【关键词】 良性阵发性位置性眩晕；360°滚转复位法；管石复位法【中图分类号】 R764.3, R764.08【文献标识码】 A【文章编号】 1672-2922（2009）02-0142-04360-degree maneuver for benign paroxysmal positional vertigoSHAN Xi-zheng, SUN Qing, MA Li-tao, LI Na, LI Hong-yun, ZHAO Long-zhu.Department of Otorhinolaryngology, General Hospital of Chinese People’s Armed Police Forces, Beijing 100039, China Corresponding author: SHAN Xi-zheng, Email: sxzent@163.com【Abstract】 ObjectiveTo determine whether 360 degree rotation in the plane of the posterior semicircular canal iseffective in treating classic benign paroxysmal positional vertigo. Methods Forty six cases of benign paroxysmal position-al vertigo in the plane of the posterior semicircular canal were reviewed. Of these cases, 25 were treated with the 360-de-gree maneuver, and 21 with the Epley maneuver. The treatment outcomes of the two maneuvers were compared. ResultsMost patients （23 in the 360-degree maneuver group and 19 in the Epley maneuver group） underwent 1 session of treat-ment, and only a few （2 in the 360-degree maneuver group and 2 in the Epley maneuver group） received 2 sessions of treatment. The treatment outcomes were not statistically different between the two groups as demonstrated by chi-square（χ2） test （α = 0.05， χ2 = 0.1173， P = 0.7319）. ConclusionThe 360-degree maneuver can be effectively used in benignparoxysmal positional vertigo in the plane of the posterior semicircular canal.
