30例胎盘脐带入口异常的产前超声诊断分析 　　[摘要] 目的 分析胎盘脐带入口异常的产前超声诊断。 方法 30例孕妇采取仰卧位和侧卧位，进行多个角度和切面的观察。采用二维超声进行常规观察，找到胎盘脐带入口，利用彩色多普勒对脐血管深入胎盘实质和脐血管进入羊水内的部位进行追踪观测，对脐带入口的准确位置进行定位。边缘脐带入口的病例测量脐带入口与胎盘边缘的距离，进一步确定有无血管前置的状况，仪器采用ALOKA-10、ALOKA-3500。 结果 胎盘脐带入口异常30例，其中，中期妊娠16例，晚期妊娠14例；边缘脐带入口共21例，帆状脐带入口共9例；胎盘附着于后壁的8例，胎盘附着于前壁侧壁的22例。 结论 产前超声诊断在胎盘脐带入口异常的检测和诊断中有重要的临床价值，能更好地指导胎盘脐带入口异常的临床治疗。 　　[关键词] 胎盘脐带入口异常；产前诊断；病理分析 　　[中图分类号] R445.1 [文献标识码] B [文章编号] 1674-4721（2013）07（a）-0088-02 　　Analysis of prenatal ultrasound diagnosis of 30 pregnant women with abnormal umbilical cord entrance in placenta 　　FENG Zhi-yi 　　Department of Ultrasound，Peoples Hospital of Heshan City in Guangdong Province，Heshan 529700，China 　　[Abstract] Objective To analyze the prenatal ultrasound diagnosis of abnormal umbilical cord entrance in placenta.Methods Thirty cases of pregnant women in supine and lateral position were routinely observed by dimensional ultrasound in multiple angles and sections to find the umbilical cord entrance in placenta.The positions where the umbilical vessels embed the placental parenchyma and amniotic fluid were tracked by color Doppler to locate the exact location of the umbilical cord entrance.The distance，from the umbilical cord entrance to the edge of placenta in patients with edge entrance of umbilical cord in placenta，were measured to further determine whether the umbilical vessels were preposing.The apparatus of ALOKA-10 and ALOKA-3500 were applied.Results Thirty cases of abnormal umbilical cord entrance in placenta were found by color Doppler detection，including 16 cases of medium-term pregnancy，14 cases of late pregnancy，21 cases of edge entrance of umbilical cord in placenta，9 cases of fan-shaped entrance of umbilical cord，8 cases of placenta attached to the posterior wall，and 22 cases of placenta attached to the anterior side wall.Conclusion Prenatal ultrasound diagnosis has important clinical value in the detection and diagnosis of entrance of abnormal umbilical cord entrance in placen
