Contractor: Insert the name of the company responsible for the job hazard analysis. Risk Subject/No. This should identify which contractors package is being assessed, e.g. steel erectors, scaffolders, etc., and the reference number of the generic job hazard analysis Prepared by; Signed Insert the name of the individual who has carried out the job hazard analysis. Date Insert the date of job hazard analysis. Activity/element The specific element under the job hazard analysis should be named. e.g. scaffolding, excavation etc. Potential Hazards The hazards which arise out of the element under job hazard analysis should be listed, e.g. sharp edges, falls etc. Population at risk The same hazard may represent different degrees of risk to different persons, e.g. on-site workers may not be at as much risk from site vehicles as passing members of the public. The different risks will need different control measures as appropriate, e.g. on-site workers may be given high visibility jackets and informed of vehicle routes, whereas traffic controllers may need to be put at the entrance to a site to protect members of the public.
