Decreased Hgb. 5/21-22 輸血 5/26 回家 醫療問題一:巴金森氏病 照護計畫(1/3) 疾病狀況: - 個案具手抖等症狀多年，近期拿碗筷時較不穩，常有掉落 或打破情形 - 會找不到東西，重複問話及忘記之前說過的事情等 - 有行動問題，外出需有他人陪伴 - 目前生活情況需要他人略微協助，服藥需他人提醒 目前藥物治療: 目前僅使用1種藥物: - Pk-Merz (Amantadine Sulphate) 100mg 0.5#BID * 第一次訪視 2月4日 巴金森氏症UPDRS量表報告 UPDRS Scale: Total: 41 Range(0-199) Part I: 2 Range(0-16) Part II: 7 Range(0-52) Part III(motor): 28 Range(0-108) Part IV: 4 Range(0-23) Hoehn-Yahr Scale: 2 Range(0-5) ADL Scale: 90 % Range(0-100%) UNIFIED PARKINSON‘S DISEASE RATING SCALE (UPDRS) ：0 = None; 1, 2, 3, ~4 = Severe. Hoehn-Yahr Scale: Stage 1 The main symptoms- tremor, muscle stiffness, slowness of movement and problems with posture- are only on one side of the body. Problems with balance might also appear. Stage 2 The disease will be on both sides of the body now and minor symptoms like problems with swallowing, talking and something called “facial masking” (loss of facial expression) may be noticed. Stage 3 The same symptoms of Stage 2 are still there but may be worse now. Problems with balance will now be noticed for the first time.
