急性脑卒中急性期血压调控与预后的多因素相关性研究 牛金茹1 阮晓兰2 刘春英2 摘要：目的 探讨急性脑卒中急性期血压调控与预后的相关性。方法 本研究对2005年9月—2007年6月发病72小时内入院的急性脑卒中符合入选标准的患者据单、双日随机分为A、B两组。于入院24小时内、7天、14天、3个月、6个月进行神经功能缺损评分，调控血压并记录其他影响预后的危险因素。结果 共纳入急性脑卒中患者1026例，血压与预后呈U型关系。入院时血压升高的发生率为71.2%，合并高血压组较不合并高血压组预后差。血压调控在140-150/80—90mmHg时预后最好，为U型关系的分界值。脑卒中急性期高血压是近期预后不良的独立危险因素，高血糖、白细胞升高与预后不良呈正相关；3-7天的血压是6个月死亡/残疾的独立影响因素。结论 急性脑卒中患者急性期血压调控是早期预后、远期预后的独立影响因素。 关键词：急性脑卒中 急性期 血压调控 预后 The acute brain died the acute time blood pressure regulation and the pre after relevant research Niu Jinru, Ruan Xiaolan, Liu Chunying ABSTRACT Objective Discusses the acute apoplexy acute time blood pressure regulation and the early pre-after relevance.Method This research June,2007 arises to from September,2005 to the acute apoplexy which in 72h is hospitalized to the selected standard patient according to the list,the even-numbered daysstochastically divides into A, the B two groups.To is hospitalized in 24h,7d,14d ,3m,6m carries on the nurve function damage.to grade,the regulative blood pressure and record other affect pre-after the dangerous factor.Result Altogether brings into line with the acute apoplexy patient 1026 examples,the blood pressure pre-after assumes the U relations.In hospitalized when the blood pressure elevates the formation rate is 71.2%,after merges the hypertension group comparatively not to merge the hypertension group pre-after to miss.Blood pressure regulation after 140-180/80-90mmHg pre-after is best,is the U relations dividing line value.After the apoplexy acute time hypertension will be the short- term pre-after not good independence dangerous factor,the high blood sugar,the white blood cell will elevate pre-after not good presents the correlation；3-7d blood presure is the independent influence factor of 6m death/deformity .Conclusion The acute apoplexy patient acute time blood pressure regulation is the early and forward pre-after independent influence factor.
