颅内动脉瘤施行早期动脉瘤夹闭术的效果观察 　　[摘要] 目的 探讨颅内动脉瘤施行早期动脉瘤夹闭术的效果。 方法 选取2012年9月～2013年12月本院就诊颅内动脉瘤患者85例，所有患者随机分为夹闭组40例（39个动脉瘤）及栓塞组45例（47个动脉瘤）。观察两者患者的效果评价、住院时间、医疗费用、完全闭塞率及并发症发生情况。 结果 两组患者的GOS、mRS评分中良好率与死亡率比较，差异无统计学意义（P0.05）。夹闭组患者的住院时间、医疗费用均少于栓塞组，两组比较差异有统计学意义（P0.05）。夹闭组完全闭塞率高于对照组（P0.05）;但术中脑血管瘤破裂率高于栓塞组（P0.05）。夹闭组患者的出血破入脑室、术后短暂性脑出血、蛛网膜下腔出血的发生率皆明显低于栓塞组，两组比较差异有统计学意义（P0.05）。 结论 临床上显微手术夹闭可作为治疗颅内动脉瘤的首选方法，其疗效显著，值得在基层医院推广应用。 　　[关键词] 颅内动脉瘤;早期动脉瘤夹闭术;效果 　　[中图分类号] R739.41 [文献标识码] B [文章编号] 2095-0616（2015）02-156-03 　　Effect observation of early aneurysm clipping operation performed on intracranial aneurysms 　　ZHANG Haiyang 　　Department of Third District Surgical，Peoples Hospital of Puning City，Puning 515300，China 　　[Abstract] Objective To study the effect of early aneurysm clipping operation performed on intracranial aneurysms. Methods 85 patients with intracranial aneurysms who were selected from September 2012 to December 2013 in our hospital were randomly divided clipping group cases and embolization group.There were 39 aneurysm in 40 patients clipping group，while 47 aneurysm were in 40 patients embolization group.To observe the effect evaluation，the hospitalization time，the medical cost，the complete obliteration rate and the occurrence of complications of the two group. Results Good rate and mortality in GOS and mRS scores had no statistical significance between the two group（P0.05）.The effect evaluation and the medical cost of clipping group were less than embolization group，difference was significant（P0.05）.The complete obliteration rate of clipping group was higher compared with embolization group（P0.05）;while the rate of cerebral aneurysm rupture in operationwas higher than embolization group（P0.05）. Conclusion Microsurgery in clapping could be used as the preferred method in the treatment of intracranial aneurysm，have significant effect and is worthy to promote and apply in primary hospitals. 　　[Key words] Intracranial aneurysm;Early aneurysm clipping operation;Effect 　　颅内动脉
