社康護士家訪時注意事項 家訪時切勿背對病人，病人及家屬的活動應在視線範圍內，以觀其動靜。 護士應坐在靠近門口的位置，掀開鐵閘上的布簾，便於呼救及引起鄰居注意。 留意病人、家屬及鄰居的行為舉止和情緒表現，如有可疑，應即與中心聯絡及商量對策。 若探訪地點周圍環境不利單獨出入，可要求家屬陪伴，如家人未能提供協助，與護士長和同事聯繫商量對策。 護理袋可放於病人及護士之間，方便突發事件發生時可起阻擋作用。 利用隨身物品，如雨傘等可起阻擋作用。 出入病人居所，保持與鄰居和護衛員打招呼，以便需要時求助。 Community Nursing Service MISSION STATEMENT As an integral part of the total health care delivery service, community nursing service provides comprehensive and individualized nursing care to people in their own environment with the aim of maximizing or maintaining their optimum level of self-care and functioning. PHILOSOPHY Community nursing is a specialty within nursing which is hospital-based and community-driven. The community is the center of interest and the recipient of nursing services. The community nurses, in collaboration with other health care team members, are responsible for an optimum level of care in the clients own environment. Each client is viewed as a unique individual with a right to a sense of dignity and worth. The clients must be cared for with an appreciation of their wholeness, integrity and relation to their family and environment regardless of nationality, race, creed or status. Care may include clients and family education, health promotion and rehabilitation. Community nurses aim at assisting clients to achieve and maintain an optimum level of self-care and functioning. Community nurses are at the forefront of care in the clients’ own environment. Community nursing requires a careful decision-making process, founded upon a sound knowledge base and the ability to assess, intervene and evaluate. This places demands on the nurses to maintain professional competence through on-going education and research.
