应用乳腺旋转瓣固定矫正轻中度乳房下垂 　　[摘要]目的：介绍采用乳房外下象限乳腺旋转瓣固定矫正轻中度乳房下垂的方法和经验。方法：经乳晕周缘“双环形”切口，去除内外环之间的表皮。在皮下脂肪组织与腺体之间广泛分离。在乳房下方正中垂直剖开乳腺，并向外侧分离，形成乳房外下象限乳腺瓣，并向内上方旋转固定，缩小乳房基底，重塑乳房外形，并上提下垂的乳头乳晕复合体。伴有明显乳腺萎缩者，同期或Ⅱ期行胸大肌下假体隆乳术。结果：2011年2月～2013年12月采用该方法共治疗轻中度乳房下垂19例，无血肿、感染、乳头乳晕坏死等并发症发生，术后随访1～2年，无乳房下垂复发，乳房下垂明显矫正，双乳对称，外形良好。切口瘢痕呈环线状，乳头乳晕感觉功能正常。结论：乳腺旋转瓣固定法结合了“双环法”和“垂直法”两种技术的优势，对轻中度乳房下垂的治疗，提供了一种新的思路和方法。 　　[关键词]乳腺瓣；乳房下垂；乳房成形术 　　[中图分类号]R655.8 [文献标示码]A [文章编号]1008-6455（2014）07-0521-03 　　 　　Correction of mild and moderate breast ptosis with rotating glandular flap fixing 　　TANG Xin-hui1,JI Ying2,SHI Wen-guo1,ZHAO Zheng-jie1,YU Chuang-ran1 　　(1.Dongguan Myfun Aesthetic ?Plastic Hospital,Dongguan 523008,Guangdong,China;2.Guangzhou Shuguang Plastic and Cosmetic Surgery Hospital) 　　 　　Abstract:ObjectiveTo introduce a new methodology for surgical treatmentof correcting mild and moderate breast ptosis by rotating and fixing lateral and inferior glandular flap.Methods Periareolar double-circle incisions were made,and the epidermis between the incisions was removed.Uderminingwas performed widely between subcutaneous tissue and gland tissue.Vertical incision was made in the middle line of inferior portion of the gland,lateral and inferiorglandular flaps were made.Flaps were rotated upward and fixed so the basement of gland became smaller.The appearance of breast was reshaped and the nipple-areola complex was elevated. If accompanied with the gland atrophy,breast augmentation with prosthesis could be undertaken at same time or at second stage.ResultsFrom Feb 2011 to Dec 2013, 19 patients were subjected to such kind of operation for correcting mild to moderate breast ptosis.1～2 years follow-up after operation,the complications,such as hematoma,infection,necrosis of nipple-areola complex,and ptosis were not found.The outcome for correcting breast ptosis was obvious. Furthermore,two breast were symmetrical with nice shape,the incision scar around areola was fine,so was the sensation of the nipple-areol
