有机产品再认证文件资料清单 Documentation List for Organic Product Recertification （植物生产和加工） (Plant Production Processing) 申请单位应提供以下文件资料并按序号编排、装订后提交： The applicant shall submit the following documents bound in the order as below:  植物生产 Plant Production 1. 《有机产品认证申请书》 Application for Organic Product Certification 2. 《有机产品再认证调查表》（植物生产） Organic Product Recertification Questionnaire (Plant Production) 3. 营业执照副本复印件 Copy of business license duplicate 4. 上年度有机产品认证证书复印件 Copy of organic product certificate for the previous year 5. 土地使用权证明文件 Documentary evidence of land use right 6. 农户清单及农户管理制度（适用于公司/合作社+农户的组织模式） List of peasants and peasant management system (applicable to the organization pattern of company/ cooperative+ peasants) 7. 产地环境质量监（检）测报告 Environmental quality monitoring (testing) report for the place of origin 8. 外购种子或种苗的购买单据、非转基因证明及未经禁用物质处理的证明 Receipts for purchase of outsourced seeds or seedlings and GMO-free and banned substance-free proof 9. 若自制肥料，提供肥料生产过程记录 Records of fertilizer production process shall be provided in case of self-produced fertilizer. 10.农事活动记录（播种、施肥、病虫害防治等） Records of farming activities (sowing, fertilizing, prevention of diseases, insect pests and weeds, etc.) 11.所有生产投入品的购买单据、产品说明书、台帐记录（来源、购买数量、使用去向与数量、库存数量等） Purchase receipts, product manuals and standing book records (source, quantity purchased, use direction and quantity, inventory, etc.) of all production inputs 12.植物收获记录，包括品种、数量、收获日期、收获方式、生产批号等 Records of plant harvesting, including species, quantity, date and method of harvest, batch number and etc. 13.销售记录 Sales records 14.有机标识的使用管理记录 Records of organic label use and management 15.培训记录（时间、培训内容、参与人员、授课人员） Training records (time, content, participants and teachers) 16.内部检查记录 Internal inspection records 17.产品召回记录 Product recall record 18.客户投诉处理记录 Records of treating customer complaints 有机产品加工（适用时） Organic Product Processing (if applicable) 对于收获后需进行加工的产品（以QS证为准），还需同时提交以下资料。 The documents as mentioned below shall be provided additionally, where the
