健康教育及管理在社区老年慢性病防治中的应用与效果 　　[摘要]目的 探讨健康教育及管理在社区老年慢性病防治中的应用与效果。方法 选取2014年4月～2015年4月社区内老年慢性病患者100例作为研究对象，将所有患者随机分为观察组（50例）和对照组（50例）。对照组采用社区内的日常护理模式；观察组在对照组基础上，采用健康教育及管理的模式。一年之后，选取所有参与调查的患者进行问卷调查，对其疾病相关知识的掌握情况、预防疾病事项知晓率、健康生活方式养成率及并发症发生率进行调查。结果 观察组对疾病知识掌握情况、对良好生活方式的养成情况、对疾病知晓率以及对并发症的发生情况均明显优于对照组，差异有统计学意义（P0.05）。观察组血压达标、血脂达标、血糖达标以及非药物治疗知晓等临床指标情况均明显优于对照组，差异有统计学意义（P0.05）。结论 对社区中患慢性病的老年人进行健康教育及管理，在疾病防治与监控中具有非常好的应用效果及应用价值，值得推广应用。 　　[关键词]健康教育及管理；社区老年慢性病；防治应用；效果研究 　　[中图分类号] R473.2 [文献标识码] A [文章编号] 1674-4721（2016）07（a）-0192-03 　　[Abstract]Objective To study the application and effects of health education and management in chronic disease prevention and treatment of elderly in community.Methods 100 elderly patients with chronic diseases from April 2014 to April 2015 of community were selected as study group.All objects were randomly divided into observation group （50 cases） and control group （50 cases）.Patients in control group were used community daily nursing mode，and patients in observation group were used health education and management mode on the basis of community daily nursing mode.1 year later，questionnaire investigation was conducted in all of patients involved in the investigation，andthe grasp situation of disease related knowledge，awareness rate of disease，cultivation of good life style and the incidence rate of complications were included.Results The rasp situation of disease related knowledge，awareness rate of disease，prevention cultivation of good life style and the incidence rate of complications in observation group were obvious better than that of control group，and the difference was statistical significance（P0.05）.The clinical index of blood pressure standard，blood lipid standard and non drug therapy awareness in observation group were obvious better than that of control group，and the difference was statistical significance when compared between two groups（P0.05）.Conclusion Conducting health education and management for elderly people with chronic disea
