Strategic focus of HEAL 17 Continue to advance New York’s health information infrastructure, based on clinical and programmatic priorities and specific goals for improving quality, affordability and outcomes Expand on implementation of health information infrastructure as an underpinning to improved coordination of patient care leveraging the Patient Centered Medical Home (PCMH). Particular focus on increasing participation by mental health, long term care and home health care providers. Five Components $120 million to improve care coordination and management through a Patient Centered Medical Home supported by Health Information Infrastructure - RGA $12 million to advance the operations of the Statewide Health Information Network for New York (SHIN-NY) $4 million to support collaboration process $2 million to develop a framework for RHIO Accreditation $2 million to support evaluation of projects Framework for New York’s Health Information Infrastructure HEAL 17 RGA Requirements Improve the coordination and management of care across the full continuum of care for a target patient population through a Patient Centered Medical Home (PCMH) model supported by the implementation and effective use of interoperable health IT. Applicants must provide, at a minimum, evidence of a completed and accepted application and initial evaluation for NCQA medical home certification or evidence of completed level 2 or level 3 NCQA certification for all primary care providers in the project.
