　　膝关节盘状半月板损伤的关节镜治疗体会的论文 【摘要】 目的 探讨膝盘状半月板损伤的临床特点，研究关节镜下盘状半月板损伤的治疗效果。方法 吉林省人民医院骨科自2002年9月～2005年8月，共进行关节镜下手术治疗盘状半月板损伤28例，其中男21例，女7例，年龄15～42岁，平均27.6岁，纵裂3例，水平裂3例，舌状裂4例，桶柄裂11例，放射状裂5例，混合裂2例，均在关节镜下治疗，行部分切除成形术19例，次全切除术7例，完全切除术2例。结果 术后随访2～24个月，平均9个月，治疗结果按ikeuchi膝关节评价等级评定［4］，分为优、良、可、差四级。其中优16例，良7例，可4例，优良率82.0%，均未发生并发症。结论 关节镜下行盘状半月板损伤的手术治疗，具有切除病损部分的半月板，可使手术前的症状迅速改善，术后不适症状发生较少，手术后可立即下床活动；减少应力增加，膝关节软骨的退行性病变较轻等诸多优点，是目前盘状半月板损伤比较理想的治疗方法。 【关键词】 关节镜 盘状半月板损伤 治疗 experience of arthroscope treatment to the knee discoid meniscus li guang-chun，liu gui-feng.orthopaedics department of people’s hospital of jilin province，changchun 130021，china ［abstract］ objective probe the character of the discoid meniscus injury and arthroscope treatment to it.methods from sep.2002 to aug.2005，28 patients eniscus injury are treated by the arthroscope in people’s hospital of jilin province，including 21 male patients and 7 female patients，age from 15 years old to 42 years old，average is 27.6 years old.results the patients are tracked from 2 to 24 moths average is 9 months.the results are devided into 4 grades:excellent，good，general and bad by the ikeuchi standerd.the excellent results are 16 patients and the good results are 7 patients and general results are 4 patients.all ent to discoid meniscus injury not only resect the damage past of meniscus，relieve the pain of patient but also plication of operation is femediately move after operation.the degenerative disease of knee articular chondro is trival.the arthroscope treatment to the knee discoid meniscus injury is a very ideal treatment eniscus injury；treatment 人类膝关节半月板为新月状、盘状或环形半月板均代表异常，由于其解剖形态的变异而容易损伤。.传???的治疗方法是切开关节囊行半月板全切除术，但术后关节软骨损伤发生几率高，且导致侧副韧带张力失衡等易引起膝关节退行性改变，远期将不可避免地导致创伤性关节炎［1］。随着对半月板功能的重视，关节镜应用于临床后，迅速成为膝关节病变诊断的金标准，同时也成为治疗膝关节病变的极佳手段，且有报道膝关节盘状半月板成形术已取得良效［2，3］。目前达成的共识是，对盘状半月板损伤，应力争在关节镜下手术，以便能够保留部分半月板［1，4］。目前在发达国家关节镜已成为常规骨科手术。 1 资料与方法 1.1 一般资料 本组研究2002年9月～2005年8月吉林省人民医院骨科所进行关节镜下手术治疗盘状半月板损伤28例侧，其中男21例侧，女7例侧，年龄15～42岁，平均27.6岁，纵裂3例侧，水平裂3例侧，舌状裂4例侧，桶柄裂1
