AutomatedObjectiveRoutineExaminationofOptical QualityofRigidEndoscopesinaClinicalSetting RensWientjes*,HerkeJ.Noordmans,JerineA.J.vanderEijk,HenkvandenBrink UniversityMedicalCentreUtrecht,Utrecht,TheNetherlands Abstract Rigidendoscopesdegradeduringclinicaluseduetosterilization,ionizingradiationandmechanicalforces.Despitevisual checksonfunctionalityatthedepartmentofsterilization,surgeonsarestillconfrontedwithsuboptimalinstrumentsasitis difficulttoassessthisdegradationinanobjectivemanner.Toguaranteethatendoscopeshavesufficientopticalqualityfor minimal invasive surgery, an experimental opto-electronic test bench has been developed in order to be used at the departmentofsterilization.Transmissionofilluminationfibresandlenscontrastvaluesarestoredinadatabasetoenable empirical criteria to reject endoscope for further clinical usage or to accept endoscopes after repair. Results of the test bencharegivenforaneightmonthperiod,whereatrainedoperatorperformed1599measurementson46differenttypes. Stabilityofthesystem,trendsinqualityofclinicalendoscopes,andeffectofrepairorreplacementwereassessed.Although theperiodwastooshorttodrawfirmconclusions,aslowdownwardstrendinqualityofclinicallyusedendoscopescould be observed. Also, endoscopes generally improve in quality after repair or replacement, while endoscope replacement seemstoslightlyoutperformendoscoperepair.Tooptimizethemeasurementprocess,anewsystemisbeingdeveloped requiringlessuserinteractionandmeasuringmoreopticalparametersofanendoscope.Bycommercializingthissystem,we hopethatmeasurementsatdifferenthospitalswillgiveimprovedinsightwhichacceptanceandrejectioncriteriatouseand whichfactors(usage,cleaningprotocol,andbrands)determinetheeconomiclifetimeofendoscopes.
