Methods: We carried outasearch in MEDLINE, Cochrane CentralRegister ofControlled Trials, andEMBASE database for relevantstudies.Relativerisks(RRs)with95%confidenceinterval(CI)werecalculatedtoassesstheassociationbetweenBMI andmortalityinpatientswithCOPD.Inaddition,abaselinerisk-adjustedanalysiswasperformedtoinvestigatethestrength ofthisassociation. Results:22studiescomprising21,150participantswereincludedinthisanalysis.Comparedwithpatientshavinganormal BMI,underweightindividualswereassociatedwithhighermortality(RR =1.34,95%CI =1.01–1.78),whereasoverweight (RR =0.47,95%CI =0.33–0.68)andobese(RR =0.59,95%CI =0.38–0.91)patientswereassociatedwithlowermortality.We furtherperformedabaselinerisk-adjustedanalysisandobtainedstatisticallysimilarresults. Conclusion: Our study showed that for patients with COPD being overweight or obese had a protective effect against mortality.However,therelationshipbetweenBMIandmortalityindifferentclassesofobesityneededfurtherclarificationin well-designedclinicalstudies.
