Objective:ToassesswhetherhighdietaryGLandGIincreasetheriskofCVDinmenandwomen. Methods: Alarge prospective cohort study (EPIC-MORGEN) wasconducted within the general Dutch population among 8,855menand10,753women,aged21–64yearsatbaseline(1993–1997)andfreeofdiabetesandCVD.Dietaryintakewas assessedwithavalidatedfood-frequencyquestionnaireandGIandGLwerecalculatedusingFoster-Powell’sinternational tableofGI.Informationonmorbidityandmortalitywasobtainedthroughlinkagewithnationalregistries.Coxproportional hazardsanalysiswasperformedtoestimatehazardratios(HRs)forincidentcoronaryheartdisease(CHD)andstroke,while adjustingforage,CVDriskfactors,anddietaryfactors. Results:Duringameanfollow-upof11.9years,581CHDcasesand120strokecasesoccurredamongmen,and300CHD casesand109strokecasesoccurredamongwomen.Inmen,GLwasassociatedwithanincreasedCHDrisk(adjustedHRper SDincrease,1.17[95%CI,1.02–1.35]),whilenosignificantassociationwasfoundinwomen(1.09[0.89–1.33]).GIwasnot associatedwithCHDriskinbothgenders,whileitwasassociatedwithincreasedstrokeriskinmen(1.27[1.02–1.58])butnot inwomen(0.96[0.75–1.22]).Similarly,totalcarbohydrateintakeandstarchintakewereassociatedwithahigherCHDriskin men(1.23[1.04–1.46];and1.24[1.07–1.45]),butnotinwomen. Conclusion:Amongmen,highGLandGI,andhighcarbohydrateandstarchintake,wereassociatedwithincreasedriskof CVD.
