In inflammatory bowel disease(IBD),thrombocytosisisconsideredamarkerofactivedisease;however,irondeficiencyitselfmaytriggerplatelet generation.InthisstudywetestedtheeffectofirontherapyonplateletcountsinpatientswithIBD-associatedanemia. Methods:Plateletcountswereanalyzedbeforeandafterirontherapyfromfourprospectiveclinicaltrials.Further,changes inhemoglobin,transferrinsaturation,ferritin,C-reactiveprotein,andleukocytecounts,beforeandafterirontherapywere compared. In a subgroup the effect of erythropoietin treatment was tested. The results were confirmed in a large independentcohort(FERGIcor). Results:Atotalof308patientrecordswereavailablefortheinitialanalysis.Adose-dependeddropinplateletcounts(mean 425G/Lto320G/L;p,0.001)wasfoundregardless ofthetypeofironpreparation(ironsulphate,ironsucrose,orferric carboxymaltose).Concomitanterythropoietintherapyaswellasparametersofinflammation(leukocytecounts,C-reactive protein) had no effect on the change in platelet counts. This effect of iron therapy on platelets was confirmed in the FERGIcorstudycohort(n=448,meanplateletcountsbeforeirontherapy:383G/L,after:310G/L,p,0.001). Conclusion:IrontherapynormalizeselevatedplateletcountsinpatientswithIBD-associatedanemia.Thus,irondeficiencyis animportantpathogeneticmechanismofsecondarythrombocytosisinIBD.
