GnocchiFoundationIRCCS,Milan,Italy Abstract Background:ReducedCD4TlymphocytescountscanbeobservedinHIVinfectionandinpatientsundergoingautologous haematopoietic stemcelltransplantation(ASCT). Nevertheless,whereasopportunistic infections (OI)arefrequentin HIV- infectedindividualswithlowcellcounts,OIareuncommoninASCTpatients. Methodology/PrincipalFindings:ToverifywhetherthisobservationcouldbesecondarytointrinsicHIV-correlatedTcell defects,weperformedin-depthimmunologicanalysesin10patientswithcomparableCD4countsinwhomlymphopenia wassecondaryeithertoHIV-infectionorASCT-associatedimmunosuppressivetherapyandcomparedthemtoage-matched healthysubjects.ResultsshowedthepresenceofprofoundalterationsinCD4+Tlymphocytesinbothgroupsofpatients with respect to healthy controls. Thus, a low percentage of CCR7+ CD4+ T cells and a compensative expansion of CD45RA2CCR72CD4+Tcells,areducedIL-2/IFN-ccytokineproductionandimpairedrecallantigens-specificproliferative responsesweredetectedbothinASCTandHIVpatients.Instarkcontrast,profounddifferencesweredetectedinCD8+T- cellsbetween
