Methods and Findings: A total of 43,405 postmenopausal women were included in the study. Baseline data for these womenweresourcedfromthe45andUpStudy,Australia,alargescalestudyofhealthyageing.Thesewomenreported beingpostmenopausal,havinganintactuterus,andhadnotbeendiagnosedwithhighbloodpressurepriortomenopause. OddsratiosfortheassociationbetweenMHTuseandhavinghighbloodpressurewereestimatedusinglogisticregression, stratified by age (,56years, 56–61years, 62–70years and over 71years) and adjusted for demographic and lifestyle factors.MHTusewasassociatedwithhigheroddsofhavinghighbloodpressure:pastmenopausalhormonetherapyuse: ,56years (adjusted odds ratio 1.59, 99% confidence interval 1.15 to 2.20); 56–61years (1.58, 1.31 to 1.90); 62–70years (1.26,1.10to1.44).Increaseddurationofhormoneusewasassociatedwithhigheroddsofhavinghighbloodpressure,with theeffectofhormonetherapyusediminishingwithincreasingage. Conclusions:Menopausalhormonetherapyuseisassociatedwithsignificantlyhigheroddsofhavinghighbloodpressure, andtheoddsincreasewithincreaseddurationofuse.Highbloodpressureshouldbeconveyedasahealthriskforpeople consideringMHTuse.
