Abnormal expressionofdefensinswasshownininflammatoryboweldisease(IBD),butitisnotclearwhetherthisisaprimarydefect. Weinvestigatedtheimpactofanti-inflammatorytherapywithinfliximabonthemucosalgeneexpressionofAMPsinIBD. Methodology/Principal Findings: Mucosal gene expression of 81AMPs wasassessed in 61 IBD patients before and4–6 weeksaftertheirfirstinfliximabinfusionandin12controlpatients,usingAffymetrixarrays.Quantitativereal-timereverse- transcriptionPCRandimmunohistochemistrywereusedtoconfirmmicroarraydata.ThedysregulationofmanyAMPsin colonic IBD in comparison with control colons was widely restored by infliximab therapy, and only DEFB1 expression remainedsignificantlydecreasedaftertherapyinthecolonicmucosaofIBDresponderstoinfliximab.InilealCrohn’sdisease (CD),expressionoftwoneuropeptideswithantimicrobialactivity,PYYandCHGB,wassignificantlydecreasedbeforetherapy compared to control ileums, and ileal PYY expression remained significantly decreased after therapy in CD responders. ExpressionofthedownregulatedAMPsbeforeandaftertreatment(DEFB1andPYY)co
