PrevalenceofSeropositivitytoPandemicInfluenza A/H1N1VirusintheUnitedStatesfollowingthe2009 Pandemic CarrieReed*,JacquelineM.Katz,KathyHancock,AmandaBalish,AliciaM.Fry*, fortheH1N1 SerosurveyWorkingGroup InfluenzaDivision,CentersforDiseaseControlandPrevention,Atlanta,Georgia,UnitedStatesofAmerica Abstract Background:2009pandemicinfluenzaA/H1N1(A(H1N1)pdm09)wasfirstdetectedintheUnitedStatesinApril2009and resultedinaglobalpandemic.WeconductedaserologicsurveytoestimatethecumulativeincidenceofA(H1N1)pdm09 throughtheendof2009whenpandemicactivityhadwanedintheUnitedStates. Methods:Weconductedapairofcrosssectionalserologicsurveysbeforeandafterthespring/fallwavesofthepandemic forevidenceofseropositivity(titer$40)usingthehemagglutinationinhibition(HI)assay.Wetestedabaselinesampleof 1,142serumspecimensfromthe2007–2008NationalHealthandNutritionExaminationSurvey(NHANES),and2,759serum specimenssubmittedforroutinescreeningtoclinicaldiagnosticlaboratoriesfromtenrepresentativesites. Results:Theage-adjustedprevalenceofseropositivitytoA(H1N1)pdm09byyear-end2009was36.9%(95%CI:31.7–42.2%). Afteradjustingforbaselinecross-reactiveantibody,pandemicvaccinationcoverageandthesensitivity/specificityoftheHI assay,weestimatethat20.2%(95%CI:10.1–28.3%)ofthepopulationwasinfectedwithA(H1N1)pdm09byDecember2009, including53.3%(95%CI:39.0–67.1%)ofchildrenaged5–17years. Conclusions: By December 2009, approximately one-fifth of the US population, or 61.9 million persons, may have been infectedwithA(H1N1)pdm09,includingaroundhalfofschool-agedchildren.
