Methods:Thisstudyisalongitudinalcohortstudyof122normoalbuminuricpatientswithT1Dwhowere receivingroutineclinicalcareatbaseline.Adetailedmedicalhistorywastaken,andaphysicalexaminationwas performedatbaseline.Allofthepatientswereregularlyexaminedfordiabetes-associatedcomplications.An analysisofpredictorswasperformedusingtheCoxregression. Results:Over6.81(3.59-9.75)yearsoffollow-up,50(41%)ofthepatientsdevelopedmicroalbuminuria.The incidencedensitywas6.79/100peopleperyear(95%CI5.04-8.95),andthemicroalbuminuriadevelopedafter5.9 (2.44-7.76)and11(5-15)yearsoffollow-upanddiabetesduration,respectively.AfteranindividualCoxregression, thebaselinevariablesassociatedwiththedevelopmentofmicroalbuminuriawereage,ageatdiagnosis,duration ofdiabetes,systolicanddiastolicbloodpressure,fastingglycemia,bodymassindex(BMI),totalcholesteroland triglycerideslevels,cholesterol/HDLratioandafamilyhistoryoftype2diabetes.AfteramultivariateCoxregression, theonlyindependentfactorsassociatedwiththedevelopmentofmicroalbuminuriawereBMI[HR1.12(1.03-1.21)] andcholesterol/HDLratio[HR1.32(1.05-1.67)]. Conclusions:AhigherBMIandcholesterol/HDLratioincreasedtheriskofdevelopingmicroalbuminuriainyoung patientswithT1Dafterashortfollow-up.Bothriskfactorsaremodifiableandshouldbeidentifiedearlyand followedclosely.
