Purposeofstudy:Theaimofthispaperistoshowtheresultsof18yrsprospectiveblindcomparativestudyusing wireandplateosteosynthesiswhichneededopenreductionandinternalfixationinvolvingTypeIItoTypeIV SpisselandSchrollZMCfractures. Materialsandmethods:Total80casesincludedinthestudyoutof1780ZMCcaseswhichweretreatedusing wireandplateosteosynthesisoveraperiodof18yrs,involvingonlyTypeIItoTypeIVSpisselandSchrollZMC fractures.Othertypesexcludedfromstudytopreventobserverbias.AllthefixationscarriedoutthroughStandard Dingman’sincisionusingstainlesssteel26gauzewireandtitanium1.5mmminiplatesystemundergeneral anesthesiabysinglemaxillofacialsurgeonandevaluatedbyanothermaxillofacialsurgeonwhoisblindedfor surgicalprocedureafter2and4wksoffollow-upforfacialsymmetry,woundhealing,functionalassessment (mouthopening,diplopia),andsensorydisturbance.AllthedatatabulatedinExcelsoftware(Microsoft)for statisticalanalysis.P-valuecalculatedtoknowtheSignificanceoftreatmentmodalityinallaspects. Results:Resultshowsnosignificantp-valuesindicatingboththeoperatingtechniquesareequallyefficientinthe surgicalmanagementofZMCfracture.
