AntoinetteKrupski1*,JeanneMSears1,2,JuttaMJoesch1,SharonEstee3,LijianHe3,AliceHuber4,ChrisDunn1, PeterRoy-Byrne1andRichardRies1 Abstract Background:Althoughbriefintervention(BI)foralcoholandotherdrugproblemshasbeenassociatedwith subsequentdecreasedlevelsofself-reportedsubstanceuse,thereislittleinformationintheextantliteratureasto whetherindividualswithco-occurringhazardoussubstanceuseandmentalillnesswouldbenefitfromBItothe sameextentasthosewithoutmentalillness.Thisisanimportantquestion,asmentalillnessisestimatedto co-occurin37%ofindividualswithanalcoholusedisorderandinmorethan50%ofindividualswithadruguse disorder.Thegoalofthisstudywastoexploredifferencesinself-reportedalcoholand/ordruguseinpatientswith andwithoutmentalillnessdiagnosessixmonthsafterreceivingBIinahospitalemergencydepartment(ED). Methods:Thisstudytookadvantageofanaturalisticsituationwhereascreening,briefintervention,andreferralto treatment(SBIRT)programhadbeenimplementedinninelargeEDsintheUSstateofWashingtonaspartofa nationalSBIRTinitiative.AsubsetofpatientswhoreceivedBIwasinterviewedsixmonthslateraboutcurrent alcoholanddruguse.Linearregressionwasusedtoassesswhetherchangeinsubstanceusemeasuresdiffered amongpatientswithamentalillnessdiagnosiscomparedwiththosewithout.Datawereanalyzedforbotha statewide(n=828)andsingle-hospital(n=536)sample. Results:Nosignificantdifferenceswerefoundbetweenmentallyillandnon-mentallyillsubgroupsineithersample withregardtoself-reportedhazardoussubstanceuseatsix-monthfollow-up. Conclusion:TheseresultssuggestthatBImaynothaveadifferingimpactbasedonthepresenceofamental illnessdiagnosis.Giventhehighprevalenceofmentalillnessamongindividualswithalcoholandotherdrug problems,thisfindingmayhaveimportantpublichealthimplications.
