Rolfastetal.CriticalCare2012,16:R100 /content/16/3/R100 RESEARCH OpenAccess Electrocardiographicchangesintherapeutic hypothermia CorinaLRolfast,ErikJLustandCarelCdeCock* Abstract Introduction:Duringtherapeutichypothermia(TH),electrocardiographic(ECG)abnormalitiessuchasOsborn wavesand/orST-segmentelevationhavebeendescribed.However,theincidenceandprognosticvalueofthese ECGchangesareuncertaingiventhesmall-scalestudiesthathavebeencarriedouttodate.Theaimofthisstudy istofurtherevaluatetheelectrocardiographicchangesduringTH. Methods:Duringaperiodof3years,81patients(age63±14years)wereincludedretrospectively.Allpatients underwentTHafterbeingresuscitated.ECGregistrationsbefore,duringandafterTHwerecollectedandanalyzed. PatientsweredividedintotwogroupsbasedonthepresenceorabsenceoftransmuralischemiaSTelevationon thefirstrepresentativeECGuponarrivalatthehospital(ST-segmentelevationmyocardialinfarction(STEMI)and non-STEMI). Results:Atotalof243ECGswereanalyzed.DuringTH24patients(30%)hadOsbornwaves,whichdisappearedin 22patients(92%)afterregainingnormalbodytemperature.ThepresenceofOsbornwaveswasnotassociated withage,gender,averagepH,electrolytes,orlactatelevelsandwasnotassociatedwithexcessin-hospital mortality.In10patients(12%,sixnon-STEMIpatients)newSTEMIwasobservedduringTH,whichdisappearedafter THdiscontinuation.TheSTEMIgroup(44patients)hadsignificantlymoreOsbornwavesduringTHthanthenon- STEMIgroup(38.6%vs.15.2%,oddsratio=3.508;95%confidenceinterval=1.281to9.610). Conclusions:Hypothermia-inducedOsbornwavesarerelativelycommonandarenotassociatedwithan unfavorableshort-termoutcome.THisassociatedwithECGchangesthatmaymimicSTEMI.
