Abstract karamanoglu@ 1NTDResearch,MedtronicInc, Minneapolis,Minnesota,USA Background:Cardiacoutput(CO)isanimportantdeterminantofthehemodynamic stateinpatientswithcongestiveheartfailure(CHF).Wetestedthehypothesisthat Fulllistofauthorinformationis availableattheendofthearticle COcanbeestimatedfromtherightventricular(RV)pressurewaveforminCHF patientsusingapulsecontourcardiacoutputalgorithmthatconsidersconstantbut patientspecificRVoutflowtractcharacteristicimpedance. Method:In12patientswithCHF,breath-by-breathFickCOandRVpressure waveformswererecordedutilizinganimplantablehemodynamicmonitorduringa bicycleexerciseprotocol.Thesedatawereanalyzedretrospectivelytoassesschanges incharacteristicimpedanceoftheRVoutflowtractduringexercise.Fourpatients thatwereimplantedwithanimplantablecardiacdefibrillator(ICD)implementingthe algorithmwerestudiedprospectively.Duringatwostagedsub-maximalbicycle exercisetestconductedat4and16weeksofimplant,COsmeasuredbydirectFick techniqueandestimatedbytheICDwererecordedandcompared. Results:Atrestthetotalpulmonaryarterialresistanceandthecharacteristic impedancewere675±345and48±18dyn.s.cm-5,respectively.Duringsub-maximal exercise,thetotalpulmonaryarterialresistancedecreased(Δ91±159dyn.s.cm-5, p0.05)butthecharacteristicimpedancewasunaffected(Δ3±9dyn.s.cm-5 ,NS). ThealgorithmderivedcardiacoutputestimatescorrelatedwithFickCO(7.6±2.5L/min, R2 = 0.92) with alimit of agreement of 1.7 L/min and tracked changes in Fick CO (R2 = 0.73). Conclusions:Theanalysisofrightventricularpressurewaveformscontinuously recordedbyanimplantablehemodynamicmonitorprovidesanestimateofCOand mayproveusefulinguidingtreatmentinpatientswithC
