Methods:SouthAsians(≥45years)inCalgary,Albertaunderwentopportunisticcardiovascularriskfactorscreening bylaytrainedvolunteersatlocalreligiousfacilities.Thosewithelevatedbloodpressure(BP)or≥1riskfactor underwentpointofcarecholesteroltesting,10-yearCVDriskcalculation,counseling,andreferraltofamily physiciansandlocalculturallytailoredchronicdiseasemanagement(CDM)programs.Participantswereinvitedfor re-screeningandweresurveyedabouthealthsystemfollow-up,satisfactionwiththeprogramandsuggestionsfor improvement.ChangesinriskfactorsfrombaselinewereestimatedusingMcNemar’stest(proportions)andpaired t-tests(continuousmeasures). Results:Baselineassessmentwascompletedfor238participants(medianage64years,51%female).MeanTC,HDL andTC/HDLwere5.41mmol/L,1.12mmol/Land4.7,respectively.Meansystolicanddiastolicbloodpressures (mmHg)were129and75respectively.BloodpressureandTC/HDLratiosexceededrecommendedtargetsin36% and58%,respectively,and76%wereathighriskforCVD.Ninety-nineparticipants(47%female)attendedre- screening.82%hadaccessedhealthcareproviders,22%reportedmedicationchangesand3.5%hadattendedthe CDMprograms.WhileBPremainedunchanged,TCandTC/HDLdecreasedandHDLincreasedsignificantly(mean differences:-0.52mmol/L,-1.04and+0.07mmol/L,respectively).Participantswereverysatisfied(80%)orsatisfied (20%)withtheproject.Participantssuggestedscree
