D-二聚体测定在急性脑梗塞中的临床应用 作者：庄文芳 左雪梅 徐伟红 刘朔 李柏胜 沈思彦 【关键词】 D-二聚体 　　【摘要】 目的 探讨血浆D-二聚体（DD）含量变化对老年急性脑梗塞患者早期诊断及病程 发展 的临床价值。 方法 采用全自动血凝 分析 透射光免疫浊度法对110例老年急性脑梗塞患者，发病1周、2周及4周和60例对照组进行血浆D-二聚体含量测定，同时检测凝血酶原时间（PT）和活化部分凝血活酶时间（APTT），并进行对比观察。结果 脑梗塞患者血浆D-二聚体含量在不同时段均明显高于对照组（P0.02或P0.001），尤其在第2周时高达（496±50）μg/L，较第1周和第4周亦明显增高（P0.05），而PT和APTT无显著性变化（P0.05）。结论提示血浆D-二聚体含量变化对早期诊断老年脑梗塞的发生及了解脑梗塞的进程有一定的 参考 价值，全自动血凝分析仪检测D-二聚体具有及时、快速、准确和简便等优点，可在临床诊断中推广 应用 。 　　关键词 脑梗塞 D-二聚体 透射光免疫浊度法 　　Clinical application of plasma D-dimer in elderly patients with acute cerebral infarction 【Abstract】 Objective To investigate the changes of plasma D-dimer in elderly patients with acute cerebral infarction and explore the clinic significance.Methods The plasma D-dimer content was determined by turbidimetˉric immunoassay at different time（1st，2nd and4th week）in110acute cerebral infarction patients（cerebral infarction group）and60normal persons（control group）.PT and APTT values were also assayed.The data was treated with t-test.Results The plasma D-dimer content in cerebral infarction group was higher than that in control group（P0.02or P0.001），and the plasma D-dimer content at2nd week was much higher than that at1st and4th week in cerebral infarction group（P0.05）.PT and APTTvalues were no significant（P0.05），and there also were no changes at different times.Conclusion The plasma D-dimer content may be considered as a marker for the early diagnosis and has certain value in distinction at different phases of the elderly patients with acute cerebral infarction.With simplicity，rapidity and accuracy，turbidimetric immunoassay using automated blood coagulation analyzer is suitˉable for determination of plasma D-dimer in clinical laboratory diagnosis.
