成人活体肝移植71例报道 作者：夏强 张建军 李齐根 张明 徐宁 陈小松 宋玉 薛峰 【摘要】 目的 探讨开展成人活体肝移植初始阶段如何确保供、受者安全。 方法 回顾性 分析 我院2007年4月至11月71例成人活体肝移植供、受者临床资料，分析评估方案、手术策略和并发症。结果 切取供者右半肝68例，其中带肝中静脉4例；切取带肝中静脉左半肝3例。术后出现并发症2例，1例胆漏，1例腹腔内出血，无供者死亡。受者外科并发症18例，其中胆道并发症12例，血管并发症3例，小肝综合征3例；病死率为10%（7/71）。结论 在开展成人活体肝移植的初始阶段，采用严格的供、受者评估、选择合理的手术方式和术后处理可以最大程度地保证供、受者安全。 【关键词】 肝移植； 活体； 成人 【Abstract】 Objective To investigate the method of securing donors and recipients during the initial procedure of adult-to-adult living donor liver transplantation （ALDLT）. Methods The clinical data, preoperative assessment, surgical strategies and complications of 71 adult donors and recipients who underwent ALDLT from April 2007 to November 2007 were retrospectively analyzed. Results Sixty-three right lobes without middle hepatic vEin （MHV）, 1 extended right lobe, 4 right lobes with MHV and 3 left lobes with MHV were obtained. Two donors suffered from postoperative complications including 1 with bile leakage and 1 with abdominal bleeding. No donor mortality occurred. EIghteen recipients had postoperative complications including 12 with biliary complications, 3 with vascular complications and 3 with small-for-size syndrome. The perioperative mortality rate of recipients was 10% （7/71）. Conclusions Strict donor and recipient assessment, optimal surgical strategy and postoperative care are extremely helpful to secure donors and recipients during the initial procedure of ALDLT. 【Key words】 Liver transplantation; Living donor; Adult 随着肝移植技术的不断 发展 和提高，越来越多的终末期肝病患者得以挽救。然而，日益紧缺的尸体供肝来源已成为我国大量等待肝移植 治疗 患者的最大障碍。近年来，活体肝移植技术在我国迅速发展。本文 总结 我院2007年4月至11月开展的71例成人活体肝移植临床资料，对受者适应证选择、供者筛选、外科手术方案和术后并发症进行分析和探讨。 　　1 资料和方法 　　1.1 供受者一般情况 71例患者中男60例，女11例；中位年龄47岁（18～63岁）。良性终末期肝病46例（65%），恶性肿瘤25例（35%）。Child A级9例， B级21例， C级41例。肝细胞癌24例，符合Milan标准的16例，超过Milan标准但符合UCSF标准的3例，超过UCSF标准但无血管侵犯、淋巴结和远处转移的5例。11例（15%）有1次以上上腹部手术史。 供者男57例， 女14例， 中位年龄25岁（20～48岁）。供、受者关系中亲属关系（包括配偶、三代内直系亲属和三代内旁系血亲）58例，非血缘亲属帮扶关系13例。所有供、受者均被要求提供亲属关系公证证明并签署手术和相关 法律 知情同意书。ABO血型完全相同67例，匹配但不相同4例（O→B
