泮托拉唑治疗消化性溃疡合并上消化道出血的临床观察 【摘要】 目的 研究 泮托拉唑钠冻干粉剂注射剂对消化性溃疡合并上消化道出血的疗效。 方法 回顾 分析 我院2003～2005年间收治的87例经内镜证实的消化性溃疡合并上消化道出血患者随机分为两组，其中泮托拉唑 治疗 组56例，奥美拉唑治疗组31例。分别以泮托拉唑及奥美拉唑40mg静滴每日2次。 结果 泮托拉唑组52例(92.86%)于4天内止血,疗效判断为显效；4例（7.14%）于6天内止血，疗效判断为有效；无效0例，总有效率达100%。奥美拉唑组于4天内止血的患者29例(93.55%), 疗效判断为显效；2例（6.45%）于6天内止血，疗效判断为有效；无效0例，总有效达100%。两组在治疗方面其显效率及有效率方面差异均无显著性（P0.05）。结论 泮托拉唑是治疗消化性溃疡合并上消化道出血的有效药物。 【关键词】 消化性溃疡；上消化道出血；泮托拉唑 　 A clinical study on intravenous pantoprazole for the management of peptic ulcer complicating with upper digestive tract bleeding 【Abstract】 Objective To investigate the effect of intravenous pantoprazole for the management of peptic ulcer bleeding.Methods Clinical data were retrospectively analyzed in 78 patients admitted to Shi Zuishan Central Hospital from 2003to 2005. The se patients with endoscopic proved pepetic ulcer bleeding were randomized divided into two groups. Fifty-six cases were classified into pantoprazole group and 31 cases into omeprazole group,and they were given intravenous pantoprazole or omeprazole 40mg twice daily.Results In pantoprazole group,the peptic ulcer bleeding in 52 cases(92.86P%)stopped in 4 days, 4cases(7.14%)in 6days, while in omeprazole group, the bleeding in 29 cases(93.55%) stopped in 4 days, 2 cases(6.45%) in 6 days. The total effective rate for controlling peptic ulcer bleeding were both 100%in two groups, there no significant differences between two groups(P0.05).Conclusion Intravenous pantoprazole is effective in the management of peptic ulcer bleeding. 【Key words】 peptic ulcer; upper digestive tract bleeding; pantoprazole 消化性溃疡合并上消化道出血是常见的内科急症，药物治疗仍是 目前 采用的首选方法，其中抑酸治疗为主要措施。H2受体阻滞剂虽可抑制胃酸分泌，但受体的耐受性 影响 了其长期疗效，质子泵抑制为目前作用最强的抑酸药物。泮托拉唑为新型质子泵抑制，临床已广泛用于治疗消化性溃疡、反流性食管炎及胃泌素瘤等相关性疾病。我院自2003年1月起 应用 泮托拉唑治疗消化性溃疡，选取消化性溃疡合并上消化道出血的临床病例87例，对其进行了临床观察。 1 资料与方法 1.1 病例资料 87例经内镜证实由消化性溃疡引起的上消化道出血患者，随机分为两组，泮托拉唑(泮托拉唑钠为锦州九泰有限责任公司生产的冻干粉针剂)治疗组56例（其中胃溃疡11例，十二指肠球部溃疡45例），男52例，女4例，年龄23～69岁，平均44±8.1岁。奥美拉唑(奥美拉唑钠为湖南五洲通药业有限责任公司生产的针剂)治疗组31例（胃溃疡4例，十二指肠球部溃疡2
