·66 · 解放军医药杂志2015 年3 月第27 卷第3 期 Med ＆ Pharm J Chin PLA ，Vol． 27 ，No． 3 ，Mar． 2015 ·论著 · 原发性肾癌肺转移32 例报道 田志辉，李 芳，邓会岩，倪晓晨，刘月平 ［摘要］ 目的 探讨原发性肾癌肺转移的诊断和治疗方法，提高对转移性肾癌的认识。方法 对2008 年 1 月—2012 年2 月收治的32 例原发性肾癌肺转移病例资料进行回顾性分析，结合病史及影像学、病理学检查选择诊治 方式。结果 经CT 及病理学检查，肺内转移灶病变与肾原发灶的组织学形态一致，免疫组化标记提示肾源性。随访 2 年，共26 例死亡。结论 转移性肾癌的术前诊断较困难，临床需提高警惕，诊断需依靠病理学检查。该病预后差， 明确诊断后，应给予肾癌根治术及综合治疗。 ［关键词］ 肾肿瘤;肿瘤转移;疾病特征;诊断;外科手术;随访研究 ［中国图书资料分类号］ Ｒ737 ． 11 ［文献标志码］A ［文章编号］2095-140X (2015)03-0066-03 ［DOI］ 10 ． 3969 /j ． issn． 2095-140X． 2015 ． 03． 016 Pulmonary Metastasis of Primary Kidney Cancer :a Ｒeport of 32 Cases TIAN Zhi-hui ，LI Fang ，DENG Hui-yan ，NI Xiao-chen ，LIU Yue-ping (Department of Pathology ，the Fourth Hospital of Hebei Medical University ，Shijiazhuang 050011，China) ［Abstract］ Objective To explore the diagnosis and treatment of pulmonary metastasis of primary kidney cancer ， and to improve awareness of metastatic kidney cancer． Methods Clinical data of 32 cases of pulmonary metastasis of primary kidney cancer admitted during January 2010 and February 2014 was retrospectively reviewed ，and the diagnosis and treatment methods were chosen according to the case history and results of examinations of imaging and pathology． Ｒesults The CT pathology examination showed that pulmonary metastases lesions were similar to primary lesions． Immu- nohistochemical makers hinted the renal source． A total of 26 patients died during two years of follow-up． Conclusion Diagnosis of metastatic kidney cancer is preoperatively difficult and therefore it should be based on pathological examina- tion results． The disease has poor prognosis ，and radical and comprehensive treatments should be given after the disease is confirmed． ［Key words］ Kidney neoplasms ;Neoplasm metastasis ;Disease attributes ;Diagnosis ;Surgical proce
