Please indicate in your cover letter and as a footnote on the title page of your manuscript that you have prepared your manuscript in the Uniform Requirements format. Only original papers written in American English will be accepted. The acceptance and publication of a manuscript is based solely on scientific merit as determined by stringent peer review. However, owing to the competitive, controversial, or specialized nature of a manuscript, authors may provide the editor
