·1634 · 中华临床医师杂志( 电子版)2014 年 5 月第 8 卷第 9 期 Chin J Clinicians(Electronic Edition),May 1,2014,Vol.8,No.9 ·临床论著· 体外循环术后中性粒细胞膜 碱性磷酸酶的变化 张婷 张春艳 张欢欢 金媛 陈建军 杨晋 吴惠毅 【摘要】 目的 观察体外循环（CPB ）后外周血中性粒细胞膜碱性磷酸酶（NAP ）表达的变 化规律以及与白细胞（WBC ）计数、C 反应蛋白（CRP ）水平之间的关系。方法 选择连云港市第 一人民医院心脏外科 CPB 心脏手术患者 20 例为研究对象，分别于术前、术后即刻 0 h、术后24 h 、 术后 48 h 、术后 72 h、出院时留取血标本，应用血细胞分析仪进行白细胞计数、免疫比浊法检测 CRP 浓度、流式细胞术定量检测 NAP 表达。结果 NAP 表达和 CRP 浓度在 CPB 术后 0 h 显著下降，随 后显著升高，48 h 达到高峰，72 h 开始下降，出院时恢复术前水平。相关分析结果表明，NAP 的表 达与血清 CRP 呈正相关，r 为 0.957 （P ＜0.001 ），高于与WBC 计数相关性，r 为 0.608 （P ＜0.01 ）。 结论 CPB 后 NAP 表达显著升高，其变化规律与血清 CRP 基本一致，可以作为 CPB 下全身炎症反 应严重程度的一个预警指标，长时间的 NAP 升高可能预示着机体存在炎症。 【关键词】 体外循环； 流式细胞术； C反应蛋白质； 中性粒细胞碱性磷酸酶 Changes of expression of alkaline phosphatase on the surface membrane of neutrophils after cardiopulmonary bypass Zhang Ting, Zhang Chunyan, Zhang Huanhuan, Jin Yuan, Chen Jianjun, Yang Jin, Wu Huiyi.
