生殖医学杂志 2011年 1O月第 2O卷增刊 (1) “达到降调’’的理解 马文红 ，梁 晓燕 (中山大学附属第六医院，广州 510655) 【摘要】 正常的生殖功能有赖于生殖神经 内分泌系统的生物节律 。促性腺激素释放激素 (GnRH)是调节促性腺激素 (Gn)合成及分泌的最主要的信号物质。体内的GnRH呈脉冲分泌，这种脉冲分泌对于生殖系统有很重要的作用 ，其脉冲频率 的调节可分别调控卵泡刺激素(FSH)及黄体生成素(LH)的合成及分泌，并能在灵长类动物中影响卵泡的发育。超生理剂量 的GnRH刺激 ，无论是通过增加频率或幅度或使用 “超活性”的GnRH激动剂 (GnRH—a)，都会抑制Gn的分泌。GnRH及 Gn— RH-a对垂体的抑制作用无区别，只是GnRH需要的剂量更大，如果每 日的注射或持续的作用代替了脉冲性的刺激，垂体 Gn 的释放将可逆性地被抑制。本文就 GnRH脉冲分泌、降调机理 、降调方案以及 口服避孕药在降调中的应用做一简单综述。 【关键词】 促性腺激素释放激素；促性腺激素释放激素激动剂； 脱敏； 卵泡刺激素； 黄体生成素； 控制性卵巢刺激 D0I：10．3969／j．issn．1004—3845．201i．z1．006 Somethingaboutpituitarydown—regulation M A Wen—hong，LIANG Xiao—yah TheSixthAfttliatedHospitalofSUN Yat—senUniversity ，Guangzhou510655 [Abstract] Successfulreproductiondependsonacomplexinterplayofbiologicrhythmsinwhichthe reproductiveneuroendocrinesystem isintimately involved．Luteinizing hormone (LH) isreleased in discreteburststhatoccuratintervalsranging from 15minutesto severalhours．Thismodeofhormone releaseisreferredtoaspulsatile，orepisodic，andistheresultofshortburstsofgonadotrophin—releasing hormone(GnRH)releasedintotheportalsystem ，with eachburstcausingarapidrisein pituitaryLH secretion．Thepituitary isabletorespondonlytransientlyto stimulation byGnRH ，andmodulationof pulse frequency providesa mechanism for differentialregulation of the synthesisand release of two hormones(LH andfollicle—stimulationhormone(FSH))from thesameceils(gonadotropes)bythesame stimulus(GnRH)．ThefrequencyofGnRH／LH pulseshasbeenshowntoaffectfolliculardevelopmentin theprimate． GnRH anditsagonistshaveadualdose—dependenteffectonthepituitarygland．Low dosesgivenina pulsatilepatternactivategonadotrophinsecretion，whilehigherdosesgraduallyblockpituitaryandgonadal functionduringrepeatedadministration．Theantifertilityeffectsobservedathigherdosesareaninherent propertyofpotentagonists，buttheyarealso observedaftersupraphysiologicaldosesofGnRH．Aftera singlehigh dose， acute stimulation of LH release is followed by a refractory pha
