中国修复重建外科杂志 2017 年 7 月第 31 卷第 7 期 • 805 •   ·骨与关节修复重建· 单根钛缆环扎结合 “8 ”字张力带治疗髌骨 骨折疗效观察 陈春永，叶君健，谢昀，林章雄，谢其扬，邱耀宇，吴贵，王发圣 福建医科大学附属第一医院创伤骨科 福建省创伤医学中心（福州   350004 ） 【摘要】   目的    探讨单根钛缆环扎结合 “8”字张力带治疗髌骨骨折的临床疗效。方法    2012 年 6 月—2014 年 11 月，收治 46 例髌骨骨折患者。男 30 例，女 16 例；年龄 20 ～86 岁，平均 54 岁。骨折原因：摔伤 27 例，交通事 故伤 16 例，击打伤 3 例。闭合性损伤 41 例，开放性损伤 5 例。左侧 24 例，右侧 22 例。骨折 AO 分型：34-A 型 3 例，34-B 型 1 例，34-C1 型 7 例，34-C2 型 13 例，34-C3 型 22 例。受伤至手术时间为 1 ～12 d，平均 3.7 d 。采用单 根钛缆环扎加 “8 ”字张力带内固定，在髌骨表面形成几何学 “球冠”形状的张力带结构。结果    术后患者切口 均 Ⅰ 期愈合。患者均获随访，随访时间 6 ～24 个月，平均 20.6 个月。术后 1 例出现软组织刺激，2 例出现 1 枚克 氏针移位、钛缆部分松动。术后 3 个月复查 X 线示骨折均已愈合，期间无钛缆断裂发生；38 例术后 12 个月时取 出内固定物。术后 12 个月，按 Böstman 髌骨骨折疗效评分标准评价为 24 ～30 分，平均 28.34 分；优 42 例、良 4 例，优良率 100% 。结论    单根钛缆环扎结合 “8 ”字张力带治疗髌骨骨折具有固定牢固、并发症少的优点，术后 允许早期进行膝关节功能锻炼，临床疗效满意。 【关键词】  髌骨骨折；张力带；钛缆；内固定 Effectiveness of cerclage and a figure-of-eight tension band by a single titanium wire in treatment of patellar fracture CHEN Chunyong, YE Junjian, XIE Yun, LIN Zhangxiong, XIE Qiyang, QIU Yaoyu, WU Gui, WANG Fasheng Department of Orthopaedic Trauma, the First Affiliated Hospital of Fujian Medical University, Trauma Center of Fujian, Fuzhou Fujian, 350004, P.R.China Corresponding author: WANG Fasheng, Email: wfshine@ 【Abstract 】 Objective    To estimate the outcome of cerclage followed by a figure-of-eight tension band with a single titanium wire for the treatment of patellar fracture.
