我院门诊2009年10月~2010年3月不合理处方分析[摘要] 目的：对我院门诊不合理处方的规范性和用药的合理性进行分析，了解我院门诊的处方质量，提高我院的医疗服务水平。方法：抽取我院门诊2009年10月～2010年3月的不合理处方4 134 张，根据《处方管理办法》、《新编药物学》、《药理学》和有关文献对不合理处方进行归类，分析其不合理原因，提出相关建议。结果：共抽查不合理处方 4 134张，以不合理总频数7 001张计，其中书写不规范的3 699张，占52.84%，用药不合理3 302张，占47.16%。结论：我院门诊仍存在一定量的不合理处方，医院、医师、药师信息部要共同努力，携手提高我院的医疗服务水平。 [关键词] 处方分析；规范性；合理性 [中图分类号] R969.3[文献标识码] C [文章编号] 1673-7210（2010）10（c）-125-03 Unreasonable prescriptions analysis in our hospital out-patient from October 2009 to March 2010 ZHU Jianxi (Department of Nine Five Clinical Equipments Division, Fuzhou General Hospital of Nanjing Military Region, Putian 351100, China) [Abstract] Objective: To investigate the norm and rationality of unreasonable prescription in the outpatients of our hospital, and understand the quality of hospital out-patient prescription, improve our hospital’s medical services. Methods: According to “Prescription Management,” “New Pharmacology”, “Pharmacology” and the relevant literatures, 4 134 unreasonable prescriptions for the outpatients in our hospital from October 2009 to March 2010 were classified, and analyzed for the irrational reasons, and some related suggestions were gave. Results: In the 4 134 random irrational prescriptions, non-standard writing was 3 699, accounting for 52.74%; irrational drug use was 3 302, accounting for 47.16%. Conclusion: There is still a certain amount of irrational prescriptions in our hospital. Therefore, hospitals, physicians and pharmacists should provide their supports and work together to improve our hospital’s medical services. [Key words] Prescriptions; Norm; Rationality 要了解临床的用药情况，必须对处方进行分析，处方分析是处理医疗纠纷的法律依据，也是促进临床合理用药的必要手段，所以处方的质量不仅与患者疾病的治疗效果息息相关，而且也直接影响医院医疗质量和管理水平，也是医院人员素质的重要体现。 1 资料与方法 1.1 一般资料 资料来源于南京军区福州总院第一附属医院2009年10月～2010年3月门诊处方，共计157 293张（剔除麻醉药品、精神药品处方），其中不合理处方共计4 134张。 1.2 方法 根据2007年5月1日新实施的《处方管理办法》中有关“四查十对”的要求[1]，检查不合理处方科室、患者姓名、年龄、处方日期、规格、数量、临床诊断和用法用量等内容[2]。以《处方管理办法》、《新编药物学》、《药理学》、《药事管理学》、药品说明书等有关文献和书籍为参考[3
