精品论文 参考文献 伴肉瘤样成分的睾丸精母细胞型精原细胞瘤临床病理分析 四川省广汉市人民医院 病理科 四川广汉 618300 【摘 要】目的：探讨伴肉瘤样成分的睾丸精母细胞型精原细胞瘤的临床病理特征、诊断及鉴别诊断，以便及时作出准确诊断，判断预后。方法：分析我院的一例典型病例、做免疫组化检测并结合文献复习。结果，伴肉瘤样成分的睾丸精母细胞型精原细胞瘤中老年多发，多半具有缓慢生长的肿瘤近期伴有加快生长的病史，镜下肿瘤由精母细胞型精原细胞瘤和肉瘤成分混杂分布却又分界截然。结论：伴肉瘤样成分的睾丸精母细胞型精原细胞瘤中老年好发，具有缓慢生长的睾丸肿块，通常有肿瘤近期加速生长的病史。肿瘤仅发生于睾丸。肿瘤具有特征性的临床及病理学特点，肿瘤罕见但诊断相对简单，预后与一般的睾丸精母细胞型精原细胞瘤不同，合并肉瘤的睾丸精母细胞型精原细胞瘤恶性程度较高，有50%以上的病例发生转移或死于该肿瘤，且所有转移肿瘤都仅含有肉瘤成分，并侵向于血道转移，转移最常发生部位为肺。 【关键词】肉瘤；精母细胞型精原细胞瘤；临床；病理 The clinical pathological analysis of spermatocyte type seminoma with sarcomatoid component XING Xiao-jun Department of pathology，Peoplersquo;s hospital of Guanghan，Sichuan 618300 Abstract：Object To discuss the clinical and pathological features， diagnosis and differential diagnosis of spermatocyte type seminoma with sarcomatoid component in order to make timely and accurate diagnosis and prognosis.Methods A typical case in our hospital had been analysed and tested immunohistochemically and then reviewed with literature.Results Spermatocyte type seminoma with sarcomatoid component was often seen in the elderly，mostly with a history of slow-growing tumors associated with a recent accelerated growth.Under the microscopic，it consisted of mixed spermatocytes type seminoma and sarcoma component but had complete boundaries.Conclusion Spermatocyte type seminoma with sarcomatoid component is often seen in the elderly，mostly with a history of slow-growing tumors associated with a recent accelerated growth.It is seen in testiculars only.It has characteristic clinical and pathological features.It is rare but relatively simple to diagnose.It has higher malignancy degree than the general permatocyte type seminoma，and over 50% of the patients have neoplasm metastasis or die of the cancer.All metastatic tumors only contain the sarcomatoid component and invade the blood channel，whose most frequent metastasis site is the lung.
