精品论文 参考文献 不同体位对机械通气长期肠内营养老年患者误吸的影响 卢丽梅 　　中国人民解放军总医院 北京 100853 　　【摘 要】目的：探讨采用4种鼻饲体位对机械通气长期肠内营养老年患者误吸的影响。方法： 对30例机械通气长期肠内营养老年患者分别采用4种鼻饲体位：①半坐卧位，置管长度从耳垂--鼻尖--剑突的距离延长10cm，推注速度为15mL／min；②仰卧位，置管长度和推注速度同上；③右侧卧位，置管长度和推注速度同上；④左侧卧位，置管长度和推注速度同上。观察采用4种体位患者误吸发生情况。结果：采用4种鼻饲体位患者发生误吸情况比较，差异具有统计学意义(Plt;0.01)。体位1误吸发生率均低于其他3种体位，差异具有统计学意义(均Plt;0.O1)。结论：鼻饲时抬高床头，采用半坐卧位，对预防机械通气长期肠内营养老年患者误吸情况发生具有一定意义。 　　【关键词】鼻饲；肠内营养；机械通气；误吸 　　【中图分类号】R472 【文献标识码】A 【文章编号】1764-8999（2015）7-0489-01 　　Different positions on the long-term enteral nutrition aspiration elderly patients with mechanical ventilation 　　Lu li-mei 　　General hospital of PLA, Beijing 100853,China 　　【Abstract】Objective：Explore the four kinds of nasal feeding position for long-term enteral nutrition aspiration elderly patients with mechanical ventilation.Methods: 30 cases of mechanical ventilation long-term enteral nutrition elderly patients with 4 kinds of nasal feeding position:①Semi-fowler,tube length from the earlobe - 10 cm is the distance from the tipndash;sword,Push the note at the rate of 15 ml/min；②The supine position,use the tube length and the injection speed up;③The right side,tube length and the injection speed up;④Left side,tube length and the injection speed up.Observe the aspiration occurred in patients with 4 kinds of postures.Results:Using four kinds of aspiration in patients with nasal feeding position, difference is statistically significant (Plt;0.01).Position 1 aspiration rates are lower than other three kinds of postures,difference is statistically significant (all Plt;0.O1).Conclusion:Nasogastric raise the head of a bed, adopting semi-fowler,mechanical ventilation to prevent long-term enteral nutrition elderly patients with aspiration case has a certain significance. 　　由于治疗的需要，机械辅助通气患者在实施营养支持时，通常需要实施肠内营养(enteralnutrition，EN)和肠外营养(parenteralnutrition，PN)，才能满足机体营养需求，一般况下首先考虑EN，PN可作为EN的一种补充途径［1］。但在经鼻胃管实施肠内营养时发生误吸的概率较高。误吸是指鼻饲液经喉头进入呼吸道，这些物质包括唾液、鼻咽部分泌物、细菌、有毒物质、胃内容物等［2］，会给患者（尤其是老年患者）带来极大的危害。 　　误吸可导致吸入性肺炎、急性呼吸窘迫综合
